From eye exams to blood tests and surgery: how doctors use light to diagnose disease

You’ve had a pounding headache all week, dizzy spells and have vomited up your past few meals.</p> <p>You visit your GP to get some answers and sit while they shine a light in your eyes, order a blood test and request some medical imaging.</p> <p>Everything your GP just did relies on light. These are just some of the optical technologies that have had an enormous impact in how we diagnose disease.</p> <h2>1. On-the-spot tests</h2> <p>Point-of-care diagnostics allow doctors to test patients on the spot and get answers in minutes, rather than sending samples to a lab for analysis.</p> <p>The “flashlight” your GP uses to view the inside of your eye (known as an <a href="https://medlineplus.gov/ency/article/003881.htm">ophthalmoscope</a>) is a great example. This allows doctors to detect abnormal blood flow in the eye, deformations of the cornea (the outermost clear layer of the eye), or swollen optical discs (a round section at the back of the eye where the nerve link to the brain begins). Swollen discs are a sign of elevated pressure inside your head (or in the worst case, a brain tumour) that could be <a href="https://www.hopkinsmedicine.org/health/conditions-and-diseases/headache/increased-intracranial-pressure-icp-headache">causing your headaches</a>.</p> <p>The invention of <a href="https://openmedscience.com/lighting-the-way-in-healthcare-the-transformative-role-of-lasers-in-medicine/">lasers and LEDs</a> has enabled many other miniaturised technologies to be provided at the bedside or clinic rather than in the lab.</p> <p><a href="https://theconversation.com/whats-a-pulse-oximeter-should-i-buy-one-to-monitor-covid-at-home-174457">Pulse oximetry</a> is a famous example, where a clip attached to your finger reports how well your blood is oxygenated. It does this by <a href="https://www.howequipmentworks.com/pulse_oximeter/">measuring</a> the different responses of oxygenated and de-oxygenated blood to different colours of light.</p> <p>Pulse oximetry is used at hospitals (and <a href="https://theconversation.com/whats-a-pulse-oximeter-should-i-buy-one-to-monitor-covid-at-home-174457">sometimes at home</a>) to monitor your respiratory and heart health. In hospitals, it is also a valuable tool for detecting <a href="https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(12)60107-X/fulltext">heart defects in babies</a>.</p> <h2>2. Looking at molecules</h2> <p>Now, back to that blood test. Analysing a small amount of your blood can diagnose <a href="https://theconversation.com/blood-tests-and-diagnosing-illness-what-can-blood-tell-us-about-whats-happening-in-our-body-80327">many different diseases</a>.</p> <p>A machine called an automated “full blood count analyser” tests for general markers of your health. This machine directs focused beams of light through blood samples held in small glass tubes. It counts the number of blood cells, determines their specific type, and reports the level of haemoglobin (the protein in red blood cells that distributes oxygen around your body). In minutes, this machine can provide a <a href="https://www.nuffieldhealth.com/article/inside-the-pathology-lab-what-happens-to-my-blood">snapshot</a> of your overall health.</p> <p>For more specific disease markers, blood serum is separated from the heavier cells by spinning in a rotating instrument called a centrifuge. The serum is then exposed to special chemical stains and enzyme assays that change colour depending on whether specific molecules, which may be the sign of a disease, are present.</p> <p>These colour changes can’t be detected with the naked eye. However, a light beam from an instrument called a <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5476943/#R88">spectrometer</a> can detect tiny amounts of these substances in the blood and determine if the biomarkers for diseases are present, and at what levels.</p> <h2>3. Medical imaging</h2> <p>Let’s re-visit those medical images your GP ordered. The development of fibre-optic technology, made famous for transforming high-speed digital communications (such as the NBN), allows light to get inside the body. The result? High-resolution optical imaging.</p> <p>A common example is an <a href="https://www.medicalnewstoday.com/articles/153737#risks-and-side-effects">endoscope</a>, where fibres with a tiny camera on the end are inserted into the body’s natural openings (such as your mouth or anus) to examine your gut or respiratory tracts.</p> <p>Surgeons can insert the same technology through tiny cuts to view the inside of the body on a video screen during <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9553337/">laparoscopic surgery</a> (also known as keyhole surgery) to diagnose and treat disease.</p> <h2>How about the future?</h2> <p>Progress in nanotechnology and a better understanding of the interactions of light with our tissues are leading to new light-based tools to help diagnose disease. These include:</p> <ul> <li> <p><a href="https://onlinelibrary.wiley.com/doi/full/10.1002/advs.201903441">nanomaterials</a> (materials on an extremely small scale, many thousands of times smaller than the width of a human hair). These are being used in next-generation sensors and new diagnostic tests</p> </li> <li> <p><a href="https://www.nature.com/articles/s41587-019-0045-y">wearable optical biosensors</a> the size of your fingernail can be included in devices such as watches, contact lenses or finger wraps. These devices allow non-invasive measurements of sweat, tears and saliva, in real time</p> </li> <li> <p>AI tools to analyse how blood serum scatters infrared light. This has allowed researchers to build a <a href="https://www.advancedsciencenews.com/powerful-diagnostic-approach-uses-light-to-detect-virtually-all-forms-of-cancer/">comprehensive database</a> of scatter patterns to detect <a href="https://onlinelibrary.wiley.com/doi/full/10.1002/aisy.202300006">any cancer</a></p> </li> <li> <p>a type of non-invasive imaging called <a href="https://www.ncbi.nlm.nih.gov/books/NBK554044/">optical coherence tomography</a> for more detailed imaging of the eye, heart and skin</p> </li> <li> <p>fibre optic technology to deliver a tiny microscope into the body on the <a href="https://www.uwa.edu.au/projects/microscope-in-a-needle">tip of a needle</a>.</p> </li> </ul> <p>So the next time you’re at the GP and they perform (or order) some tests, chances are that at least one of those tests depend on light to help diagnose disease.<!-- Below is The Conversation's page counter tag. For type 2 diabetes, focusing on when you eat – not what – can help control blood sugar

<div class="theconversation-article-body"> <p><em><a href="https://theconversation.com/profiles/evelyn-parr-441878">Evelyn Parr</a>, <a href="https://theconversation.com/institutions/australian-catholic-university-747">Australian Catholic University</a> and <a href="https://theconversation.com/profiles/brooke-devlin-2237174">Brooke Devlin</a>, <a href="https://theconversation.com/institutions/the-university-of-queensland-805">The University of Queensland</a></em></p> <p>Type 2 diabetes affects <a href="https://www.aihw.gov.au/reports/diabetes/diabetes/contents/how-common-is-diabetes/type-2-diabetes">1.2 million Australians</a> and accounts for <a href="https://www.diabetesaustralia.com.au/about-diabetes/type-2-diabetes/">85-90%</a> of all diabetes cases. This chronic condition is characterised by high blood glucose (sugar) levels, which carry serious <a href="https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(17)30058-2/abstract">health</a> risks. <a href="https://www.nature.com/articles/nrendo.2017.151">Complications</a> include heart disease, kidney failure and vision problems.</p> <p>Diet is an important way people living with type 2 diabetes manage blood glucose, alongside exercise and medication. But while we know individualised, professional dietary advice improves blood glucose, it can be <a href="https://linkinghub.elsevier.com/retrieve/pii/S0168822717317588">complex</a> and is not always <a href="https://www.publish.csiro.au/py/PY13021">accessible</a>.</p> <p><a href="https://www.sciencedirect.com/science/article/pii/S0168822724008039">Our new study</a> looked at the impact of time-restricted eating – focusing on when you eat, rather than what or how much – on blood glucose levels.</p> <p>We found it had similar results to individualised advice from an accredited practising dietitian. But there were added benefits, because it was simple, achievable, easy to stick to – and motivated people to make other positive changes.</p> <h2>What is time-restricted eating?</h2> <p>Time-restricted eating, also known as <a href="https://www.annualreviews.org/content/journals/10.1146/annurev-nutr-082018-124320">the 16:8 diet</a>, became popular for weight loss around 2015. Studies have since shown it is also an <a href="https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2811116">effective way</a> for people with type 2 diabetes to manage blood glucose.</p> <p>Time-restricted eating involves limiting when you eat each day, rather than focusing on what you eat. You restrict eating to a window during daylight hours, for example between 11am and 7pm, and then fast for the remaining hours. This can sometimes naturally lead to also eating less.</p> <p>Giving your body a break from constantly digesting food in this way helps align eating with natural <a href="https://doi.org/10.1111/jne.12886">circadian rhythms</a>. This <a href="https://doi.org/10.1111/jnc.15246">can help</a> regulate metabolism and improve overall health.</p> <p>For people with type 2 diabetes, there may be specific benefits. They often have their <a href="https://doi.org/10.2337/dc12-2127">highest blood glucose</a> reading in the morning. Delaying breakfast to mid-morning means there is time for physical activity to occur to help reduce glucose levels and prepare the body for the first meal.</p> <h2>How we got here</h2> <p>We ran an <a href="https://www.mdpi.com/2072-6643/12/11/3228">initial study</a> in 2018 to see whether following time-restricted eating was achievable for people with type 2 diabetes. We found participants could easily stick to this eating pattern over four weeks, for an average of five days a week.</p> <p>Importantly, they also had improvements in blood glucose, spending less time with high levels. <a href="https://www.mdpi.com/2072-6643/12/2/505">Our previous research</a> suggests the reduced time between meals may play a role in how the hormone insulin is able to reduce glucose concentrations.</p> <p><a href="https://doi.org/10.1001/jamanetworkopen.2023.39337">Other studies</a> have confirmed these findings, which have <a href="https://doi.org/10.1186/s12986-021-00613-9">also shown</a> notable improvements in HbA1c. This is a <a href="https://www.ncbi.nlm.nih.gov/books/NBK304271/">marker</a> in the blood that represents concentrations of blood glucose over an average of three months. It is the <a href="https://journals.sagepub.com/doi/10.4137/BMI.S38440">primary clinical tool</a> used for diabetes.</p> <p>However, these studies provided intensive support to participants through weekly or fortnightly meetings with researchers.</p> <p>While we know this level of support <a href="https://www.nature.com/articles/0802295">increases</a> how likely people are to stick to the plan and improves outcomes, it is not readily available to everyday Australians living with type 2 diabetes.</p> <h2>What we did</h2> <p>In our <a href="https://www.sciencedirect.com/science/article/pii/S0168822724008039">new study</a>, we compared time-restricted eating directly with advice from an <a href="https://dietitiansaustralia.org.au/working-dietetics/standards-and-scope/role-accredited-practising-dietitian">accredited practising dietitian</a>, to test whether results were similar across six months.</p> <p>We recruited 52 people with type 2 diabetes who were currently managing their diabetes with up to two oral medications. There were 22 women and 30 men, aged between 35 and 65.</p> <p>Participants were randomly divided into two groups: diet and time-restricted eating. In both groups, participants received four consultations across the first four months. During the next two months they managed diet alone, without consultation, and we continued to measure the impact on blood glucose.</p> <p>In the diet group, consultations focused on changing their diet to control blood glucose, including improving diet quality (for example, eating more vegetables and limiting alcohol).</p> <p>In the time-restricted eating group, advice focused on how to limit eating to a nine-hour window between 10am and 7pm.</p> <p>Over six months, we measured each participant’s blood glucose levels every two months using the HbA1c test. Each fortnight, we also asked participants about their experience of making dietary changes (to what or when they ate).</p> <h2>What we found</h2> <p>We found time-restricted eating was as effective as the diet intervention.</p> <p>Both groups had reduced blood glucose levels, with the greatest improvements occurring after the first two months. Although it wasn’t an objective of the study, some participants in each group also lost weight (5-10kg).</p> <p>When surveyed, participants in the time-restricted eating group said they had adjusted well and were able to follow the restricted eating window. Many told us they had family support and enjoyed earlier mealtimes together. Some also found they slept better.</p> <p>After two months, people in the time-restricted group were looking for more dietary advice to further improve their health.</p> <p>Those in the diet group were less likely to stick to their plan. Despite similar health outcomes, time-restricted eating seems to be a simpler initial approach than making complex dietary changes.</p> <h2>Is time-restricted eating achievable?</h2> <p>The main barriers to following time-restricted eating are social occasions, caring for others and work schedules. These factors may prevent people eating within the window.</p> <p>However, there are many benefits. The message is simple, focusing on when to eat as the main diet change. This may make time-restricted eating more translatable to people from a wider variety of socio-cultural backgrounds, as the types of foods they eat don’t need to change, just the timing.</p> <p>Many people don’t have access to more individualised support from a dietitian, and receive nutrition advice from their GP. This makes time-restricted eating an alternative – and equally effective – strategy for people with type 2 diabetes.</p> <p>People should still try to stick to <a href="https://www.eatforhealth.gov.au/guidelines/guidelines">dietary guidelines</a> and prioritise vegetables, fruit, wholegrains, lean meat and healthy fats.</p> <p>But our study showed time-restricted eating may also serve as stepping stone for people with type 2 diabetes to take control of their health, as people became more interested in making diet and other positive changes.</p> <p>Time-restricted eating might not be appropriate for everyone, especially people on medications which don’t recommend fasting. Before trying this dietary change, it’s best speak to the healthcare professional who helps you manage diabetes.<!-- Below is The Conversation's page counter tag. I’m feeling run down. Why am I more likely to get sick? And how can I boost my immune system?

<div class="theconversation-article-body"> <p><em><a href="https://theconversation.com/profiles/sathana-dushyanthen-1169328">Sathana Dushyanthen</a>, <a href="https://theconversation.com/institutions/the-university-of-melbourne-722">The University of Melbourne</a></em></p> <p>It has been a long winter, filled with many viruses and cost-of-living pressures, on top of the usual mix of work, study, life admin and caring responsibilities.</p> <p>Stress is an inevitable part of life. In short bursts, our stress response has evolved as a survival mechanism to help us be more alert in <a href="https://theconversation.com/no-you-cant-blame-all-your-health-issues-on-high-cortisol-heres-how-the-hormone-works-203162">fight or flight situations</a>.</p> <p>But when stress is chronic, it weakens the immune system and makes us more vulnerable to illnesses such as the <a href="https://www.healthline.com/health/can-stress-make-you-sick#:%7E:text=The%20common%20cold&amp;text=Inflammation%20has%20been%20linked%20to,to%20the%20cold%2Dcausing%20germs.">common cold</a>, <a href="https://journals.lww.com/psychosomaticmedicine/abstract/1999/03000/psychological_stress,_cytokine_production,_and.9.aspx">flu</a> and <a href="https://onlinelibrary.wiley.com/doi/abs/10.1002/smi.3017">COVID</a>.</p> <h2>Stress makes it harder to fight off viruses</h2> <p>When the immune system starts to break down, a virus that would normally have been under control starts to flourish.</p> <p>Once you begin to feel sick, the <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4465119/">stress response</a> rises, making it harder for the immune system to fight off the disease. You may be sick more often and for longer periods of time, without enough immune cells primed and ready to <a href="https://link.springer.com/chapter/10.1007/978-3-030-16996-1_6">fight</a>.</p> <p>In the 1990s, American psychology professor Sheldon Cohen and his colleagues conducted a number of <a href="https://www.cmu.edu/common-cold-project/">studies</a> where healthy people were exposed to an upper respiratory infection, through drops of virus placed directly into their <a href="https://www.nejm.org/doi/full/10.1056/NEJM199108293250903">nose</a>.</p> <p>These participants were then quarantined in a hotel and monitored closely to determine who became <a href="https://theconversation.com/stress-less-it-might-protect-you-from-covid-153361">ill</a>.</p> <p>One of the most important factors predicting who got sick was prolonged psychological <a href="https://journals.sagepub.com/doi/full/10.1177/1745691620942516">stress</a>.</p> <h2>Cortisol suppresses immunity</h2> <p>“Short-term stress” is stress that lasts for a period of minutes to hours, while “chronic stress” persists for several hours per day for <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5964013/#:%7E:text=Therefore%2C%20a%20major%20distinguishing%20characteristic,weeks%20or%20months%20%5B9%5D.">weeks or months</a>.</p> <p>When faced with a perceived threat, psychological or physical, the hypothalamus region of the brain sets off an alarm system. In the short term, cortisol suppresses inflammation, to ensure the body has enough energy available to respond to an <a href="https://www.frontiersin.org/journals/immunology/articles/10.3389/fimmu.2019.00245/full">immediate threat</a>.</p> <p>But in the longer term, chronic stress can be harmful. A Harvard University study <a href="https://jamanetwork.com/journals/jamapsychiatry/fullarticle/2796097">from 2022</a> showed that people suffering from psychological distress in the lead up to their COVID infection had a greater chance of experiencing long COVID. They <a href="https://jamanetwork.com/journals/jamapsychiatry/fullarticle/2796097">classified</a> this distress as depression, probable anxiety, perceived stress, worry about COVID and loneliness.</p> <p>Those suffering distress had close to a <a href="https://jamanetwork.com/journals/jamapsychiatry/fullarticle/2796097">50% greater risk</a> of long COVID compared to other <a href="https://theconversation.com/being-stressed-out-before-you-get-covid-increases-your-chances-of-long-covid-heres-why-190649">participants</a>. Cortisol has been shown to be high in the most severe cases of <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7102614/">COVID</a>.</p> <h2>Stress causes inflammation</h2> <p><a href="https://stories.uq.edu.au/imb/the-edge/inflammation/what-is-inflammation/index.html">Inflammation</a> is a short-term reaction to an injury or infection. It is responsible for trafficking immune cells in your body so the right cells are present in the right locations at the right times and at the right <a href="https://link.springer.com/article/10.1007/s12026-014-8517-0">levels</a>.</p> <p>The immune cells also store a memory of that threat to respond faster and more effectively the next <a href="https://theconversation.com/being-stressed-out-before-you-get-covid-increases-your-chances-of-long-covid-heres-why-190649">time</a>.</p> <p>Initially, circulating immune cells detect and flock to the site of <a href="https://www.nature.com/articles/ni1275">infection</a>. Messenger proteins, known as pro-inflammatory cytokines, are released by immune cells, to signal the danger and recruit help, and our immune system responds to neutralise the <a href="https://stories.uq.edu.au/imb/the-edge/inflammation/what-is-inflammation/index.html">threat</a>.</p> <p>During this response to the infection, if the immune system produces too much of these inflammatory chemicals, it can trigger symptoms such as nasal congestion and runny <a href="https://journals.sagepub.com/doi/full/10.1177/1745691620942516">nose</a>.</p> <h2>What about chronic stress?</h2> <p>Chronic stress causes persistently high cortisol secretion, which remains high even in the absence of an immediate <a href="https://theconversation.com/no-you-cant-blame-all-your-health-issues-on-high-cortisol-heres-how-the-hormone-works-203162">stressor</a>.</p> <p>The immune system becomes desensitised and unresponsive to this <a href="https://www.sciencedirect.com/science/article/abs/pii/S1043661816307435">cortisol suppression</a>, increasing low-grade “silent” inflammation and the production of pro-inflammatory cytokines (the messenger proteins).</p> <p>Immune cells become exhausted and start to <a href="https://www.fxmedicine.com.au/blog-post/adrenal-immune-connection">malfunction</a>. The body loses the ability to turn down the inflammatory <a href="https://journals.sagepub.com/doi/full/10.1177/1745691620942516">response</a>.</p> <p>Over time, the immune system changes the way it responds by reprogramming to a “<a href="https://www.unimelb.edu.au/newsroom/news/2021/april/how-stress-can-stop-immune-cells-in-their-tracks">low surveillance mode</a>”. The immune system misses early opportunities to destroy threats, and the process of recovery can take <a href="https://theconversation.com/being-stressed-out-before-you-get-covid-increases-your-chances-of-long-covid-heres-why-190649">longer</a>.</p> <h2>So how can you manage your stress?</h2> <p>We can actively strengthen our immunity and natural defences by managing our <a href="https://theconversation.com/stuck-in-fight-or-flight-mode-5-ways-to-complete-the-stress-cycle-and-avoid-burnout-or-depression-218599">stress levels</a>. Rather than letting stress build up, try to address it early and frequently by:</p> <p><strong>1) Getting enough sleep</strong></p> <p>Getting enough sleep reduces <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3132857/">cortisol levels</a> and inflammation. During sleep, the immune system <a href="https://link.springer.com/chapter/10.1007/978-1-4939-6578-6_12">releases</a> <a href="https://theconversation.com/sleep-wont-cure-the-coronavirus-but-it-can-help-our-bodies-fight-it-134674">cytokines</a>, which help fight infections and inflammation.</p> <p><strong>2) Taking regular exercise</strong></p> <p>Exercising helps the lymphatic system (which balances bodily fluids as part of the immune system) circulate and allows immune cells to monitor for threats, while sweating flushes <a href="https://www.healthline.com/nutrition/does-exercise-boost-immune-system">toxins</a>. Physical activity also lowers stress hormone levels through the release of positive brain <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7387807/">signals</a>.</p> <p><strong>3) Eating a healthy diet</strong></p> <p>Ensuring your diet contains enough nutrients – such as the B vitamins, and the full breadth of minerals like magnesium, iron and zinc – during times of stress has a positive impact on overall stress <a href="https://www.ncbi.nlm.nih.gov/pubmed/22782571">levels</a>. Staying hydrated helps the body to flush out <a href="https://theconversation.com/a-strong-immune-system-helps-ward-off-colds-and-flus-but-its-not-the-only-factor-99512">toxins</a>.</p> <p><strong>4) Socialising and practising meditation or mindfulness</strong></p> <p>These activities increase endorphins and serotonin, which improve mood and have <a href="https://www.sciencedirect.com/science/article/pii/S2949834123000351">anti-inflammatory effects</a>. Breathing exercises and meditation stimulate the parasympathetic nervous system, which calms down our stress responses so we can “reset” and reduce <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4940234/">cortisol levels</a>.<!-- Below is The Conversation's page counter tag. New support system helps older Aussies who have fallen victim to scams

<p>A new support system has been set up to help vulnerable older Australians who have fallen victim to dangerous scams. </p> <p>With millions of Aussies targeted every year, many of these targets are elderly with 30 percent of victims hesitant to come forward because they are too embarrassed to admit what's happened to them.</p> <p>One such victim, 86-year-old Mary, said she was humiliated after falling for a scam saying she is old enough to know better. </p> <p>"It serves me right, I shouldn't have been so trusting," she said.</p> <p>"You blame yourself and you don't really want to tell people how stupid you are, so you think, 'Well, I'll just have to bear it'."</p> <p>Mary was scammed when she received a call from a man pretending to be a technician, saying he needed access to her computer to resolve a problem in her area. </p> <p>Step by step, the "technician" got Mary to log-in and give him complete access to her entire online identification, completely cleaning her out of $20,000 in savings.</p> <p>This is why <a title="ID Support NSW" href="https://www.nsw.gov.au/id-support-nsw" target="_blank" rel="noopener">ID Support NSW</a> is now offering a free service to help older Australians be able to protect themselves online and spot suspicious activity.</p> <p>"The most common scams are romance and impersonation scams, where people pretend to be someone they're not," ID Support NSW's Jacqueline Wilson told <em><a href="https://9now.nine.com.au/today/scam-awareness-week-id-support-nsw-offering-free-help-for-older-australians/20d3f8d6-fe32-4b1d-af32-294e47f4412d" target="_blank" rel="noopener">Today</a></em>.</p> <p>"Most start with a text or call and once someone responds or engages, they usually end up losing money."</p> <p>Jacqueline said the best thing you could do if you even suspect a scammer was to just hang up the phone, saying, "You don't need to indulge them, you don't need to be polite, you don't need to engage in a long conversation."</p> <p><em>Image credits: Today</em> </p>

Australia’s tax system is being weaponised against victims of domestic abuse. Here’s how

<div class="theconversation-article-body"> <p><em><a href="https://theconversation.com/profiles/ann-kayis-kumar-466422">Ann Kayis-Kumar</a>, <a href="https://theconversation.com/institutions/unsw-sydney-1414">UNSW Sydney</a></em></p> <p>When women seeking financial help from the government-funded UNSW <a href="https://www.unsw.edu.au/business/our-schools/accounting-auditing-taxation/about-us/unsw-clinic">Tax and Business Advisory Clinic</a> are asked whether they have ever been affected by family or domestic violence, most say they have.</p> <p>In the past year this number has grown from <a href="https://cdn.theconversation.com/static_files/files/3339/sub009.pdf?1718777706">65%</a> to over 80%.</p> <p>And about <a href="https://ssrn.com/abstract=4746954">14%</a> of the clinic’s clients say their tax debts are a result of intimate partner violence. These debts often arise from business debts, bankruptcy, corporate directorships and director penalty notices.</p> <p>We know that economic abuse is a red flag for other forms of domestic violence. Economic abuse occurs in nearly all Australian domestic and family violence cases, affecting more than <a href="https://www.abs.gov.au/statistics/people/crime-and-justice/personal-safety-australia/latest-release">2.4 million Australians</a> and costing the economy an estimated <a href="https://www.commbank.com.au/content/dam/caas/newsroom/docs/Cost%20of%20financial%20abuse%20in%20Australia.pdf">A$10.9 billion</a> a year.</p> <p>Unfortunately, existing laws fall well short of protecting abuse victim-survivors from financial loss.</p> <h2>How violent partners weaponise tax</h2> <p>The perpetrators of violence can effectively weaponise the tax system by placing tax debts solely in the names of former partners, often because they have made them directors of companies or through family businesses operating through partnerships or trusts.</p> <p>There is a policy assumption that family members benefit from family partnerships.</p> <p>But this does not always hold in practice and can be problematic when there is economic abuse because Australian tax law requires victims report and pay tax on their “share” of the family partnership’s income.</p> <p>The average tax debt at the tax clinic is about $90,000. This can result in debilitating financial burdens, exhausted savings, insecure housing and prolonged economic instability, well after abusive relationships end.</p> <h2>Change is needed</h2> <p>Australia has no specific strategy for relief of tax debts caused by financial abuse. There are “serious hardship” provisions in Australian taxation law, but these are outdated and in need of <a href="https://theconversation.com/sometimes-people-can-do-with-a-break-3-ways-tax-debt-relief-rules-are-too-tough-156948">reform</a>.</p> <p>Usually people do not have the funds up front so the only way the Australian Taxation Office can collect debts from the abused partner is through (generally two-year) payment plans, offsetting future tax refunds, engaging external debt collectors and initiating bankruptcy proceedings.</p> <p>To that end, the decision announced in this year’s budget to give the Tax Commissioner discretion <a href="https://www.ato.gov.au/about-ato/new-legislation/in-detail/businesses/changes-to-offsetting-debts-on-hold">not to offset</a> against tax returns debts previously placed “on hold” is welcome.</p> <p>It will provide short-term relief by enabling abuse victims to get their refunds instead of having it used by the Tax Office to reduce their debt.</p> <p>Colleagues Christine Speidel, Leslie Book and I want this power extended to all <a href="https://papers.ssrn.com/sol3/papers.cfm?abstract_id=4746954">forms of tax debts</a> not just for tax debts that have been placed “on hold” especially where the taxpayer is known to have experienced financial abuse.</p> <p>But this wouldn’t go far enough – the victim-survivors would still have the perpetrator’s tax debt hanging over them.</p> <p>Where this happens, financial instability can <a href="https://www.commbank.com.au/content/dam/commbank-assets/support/2021-01/unsw-report-key-findings.pdf">drive women back</a> into abusive relationships.</p> <h2>The US shows what can be done</h2> <p>Legislative reform to shift tax liability from abuse survivors to perpetrators is the key to helping solve the problem.</p> <figure class="align-right zoomable"></figure> <p>The United States has offered some form of “<a href="https://www.irs.gov/individuals/innocent-spouse-relief">innocent spouse relief</a>” since 1971. In 2011 it widened eligibility and <a href="https://www.irs.gov/pub/irs-news/ir-11-080.pdf">removed a two-year time limit</a> for requesting relief.</p> <p>It is important to understand the US provisions apply because the country offers jointly filed “married” tax returns. In Australia tax returns are filed by individuals.</p> <p>Australia’s laws would need to change to ensure abused women do not find themselves jointly liable. Any changes should also include debts incurred in the name of partnerships and company directors.</p> <p>The US is the first and only country to do this, largely because of the advocacy of US low-income tax clinics over decades. Australia now has such clinics, funded as part of the Tax Office <a href="https://www.ato.gov.au/individuals-and-families/financial-difficulties-and-disasters/support-to-lodge-and-pay/national-tax-clinic-program">National Tax Clinic Program</a>.</p> <p>Australia’s adoption of US-style rules could provide a model for other jurisdictions, increase tax debt collection (as perpetrators are likely to have better capacity to pay than victims) and foster greater confidence in the Tax Office.</p> <p>Most importantly, it would acknowledge that victim-survivors with tax debts should not bear responsibility for debts incurred by perpetrators.</p> <hr /> <p><em>For information and advice about family and intimate partner violence contact 1800 RESPECT (<a href="https://www.1800respect.org.au/">1800 737 732</a>). Our housing system is broken and the poorest Australians are being hardest hit

<div class="theconversation-article-body"> <p><em><a href="https://theconversation.com/profiles/rachel-ong-viforj-113482">Rachel Ong ViforJ</a>, <a href="https://theconversation.com/institutions/curtin-university-873">Curtin University</a></em></p> <p>Just when we think the price of rentals could not get any worse, this week’s <a href="https://www.anglicare.asn.au/publications/2023-rental-affordability-snapshot/">Rental Affordability Snapshot</a> by Anglicare has revealed low-income Australians are facing a housing crisis like never before.</p> <p>In fact, if you rely on the <a href="https://www.servicesaustralia.gov.au/youth-allowance">Youth Allowance</a>, there is not a single rental property across Australia you can afford this week.</p> <h2>How did rental affordability get this bad?</h2> <p>Several post-COVID factors have been blamed, including our preference for <a href="https://www.rba.gov.au/publications/bulletin/2023/jun/new-insights-into-the-rental-market.html">more space, the return of international migrants</a>, and <a href="https://www.corelogic.com.au/news-research/news/2023/could-the-peak-in-interest-rates-signal-an-end-to-rising-rents">rising interest rates</a>.</p> <p>However, the rental affordability crisis pre-dates COVID.</p> <p>Affordability has been steadily declining for decades, as successive governments have failed to make shelter more affordable for low-to-moderate income Australians.</p> <h2>The market is getting squeezed at both ends</h2> <p>At the lower end of the rental sector, the growth in the supply of social housing persistently lags behind demand, trending at under <a href="https://povertyandinequality.acoss.org.au/data/annual-growth-rates-social-housing-stock-and-population-2011-2020/">one-third</a> the rate of population growth.</p> <hr /> <p><iframe id="OA0cS" class="tc-infographic-datawrapper" style="border: none;" src="https://datawrapper.dwcdn.net/OA0cS/" width="100%" height="400px" frameborder="0"></iframe></p> <hr /> <p>This has forced growing numbers of low-income Australians to seek shelter in the private rental sector, where they face intense competition from higher-income renters.</p> <p>At the upper end, more and more aspiring home buyers are getting <a href="https://www.tandfonline.com/doi/full/10.1080/03085147.2021.2003086">locked out</a> of home ownership.</p> <p>A recent <a href="https://www.ahuri.edu.au/sites/default/files/documents/2024-02/AHURI-Final-Report-416-Affordable-private-rental-supply-and-demand-short-term-disruption.pdf">study</a> found more households with higher incomes are now renting.</p> <p>Households earning <a href="https://www.ahuri.edu.au/sites/default/files/documents/2024-02/AHURI-Final-Report-416-Affordable-private-rental-supply-and-demand-short-term-disruption.pdf">$140,000</a> a year or more (in 2021 dollars) accounted for just 8% of private renters in 1996. By 2021, this tripled to 24%. No doubt, this crowds out lower-income households who are now facing a shortage of affordable homes to rent.</p> <h2>Why current policies are not working</h2> <p>Worsening affordability in the private rental sector highlights a housing system that is broken. Current policies just aren’t working.</p> <p>While current policies focus on supply, more work is needed including fixing <a href="https://theconversation.com/governments-are-pouring-money-into-housing-but-materials-land-and-labour-are-still-in-short-supply-205471">labour shortages</a> and providing greater <a href="https://theconversation.com/people-want-and-need-more-housing-choice-its-about-time-governments-stood-up-to-deliver-it-122390">stock diversity</a>.</p> <p>The planning system plays a critical role and <a href="https://theconversation.com/confusing-and-not-delivering-enough-developers-and-councils-want-new-affordable-housing-rules-139762">zoning rules</a> can be reformed to support the supply of more affordable options.</p> <p>However, the housing affordability challenge is not solely a supply problem. There is also a need to respond to the <a href="https://theconversation.com/home-prices-are-climbing-alright-but-not-for-the-reason-you-might-think-158776">super-charged demand</a> in the property market.</p> <p>An overheated market will undoubtedly place intense pressure on the rental sector because aspiring first home buyers are forced to rent for longer, as house prices soar at a rate unmatched by their wages.</p> <p>Yet, governments continue to resist calls for winding back the <a href="https://www.abc.net.au/news/2024-02-15/ken-henry-australias-tax-system-in-worse-position-after-15-years/103465044">generous tax concessions</a> enjoyed by multi-property owners.</p> <p>The main help available to low-income private renters - the Commonwealth Rent Assistance scheme - is <a href="https://theconversation.com/1-billion-per-year-or-less-could-halve-rental-housing-stress-146397">poorly targeted</a> with nearly one in five low-income renters who are in rental stress deemed ineligible, while another one in four receive it despite not being in rental stress.</p> <h2>Can affordable housing occur naturally?</h2> <p>Some commentators support the theory of <a href="https://www.ahuri.edu.au/sites/default/files/documents/2022-09/Executive-Summary-FR387-Filtering-as-a-source-of-low-income-housing-in-Australia-conceptualisation-and-testing.pdf">filtering</a> - a market-based process by which the supply of new dwellings in more expensive segments creates additional supply of dwellings for low-income households as high-income earners vacate their former dwellings.</p> <p>Proponents of filtering argue building more housing anywhere - even in wealthier ends of the property market - will eventually improve affordability across the board because lower priced housing will trickle down to the poorest households.</p> <p>However, the persistent affordability crisis low-income households face and the rise in homelessness are crucial signs filtering <a href="https://cloud.3dissue.com/122325/122578/143598/WhyNewSupplyisnotExpandingHousingOptionsfortheHomeless/html5/index.html?page=1&amp;noflash">does not work well</a> and <a href="https://www.ahuri.edu.au/sites/default/files/documents/2022-09/AHURI-Final-Report-387-Filtering-as-a-source-of-low-income-housing-in-Australia-conceptualisation-and-testing.pdf">cannot be relied upon</a> to produce lower cost housing.</p> <h2>Location, location, location</h2> <p>Location does matter, if we expect building new housing to work for low-income individuals.</p> <p>What is needed is a steady increase of affordable, quality housing in areas offering low-income renters the same access to jobs and amenities as higher-income households.</p> <p>The <a href="https://treasury.gov.au/housing-policy/accord#:%7E:text=The%20Accord%20includes%20an%20initial,5%20years%20from%20mid%E2%80%912024.">National Housing Accord</a> aims to deliver 1.2 million new dwellings over five years from mid-2024. But it must ensure these are “well-located” for people who need affordable housing, as suggested in the accord.</p> <p>Recent <a href="https://www.tandfonline.com/doi/full/10.1080/02673037.2023.229051">modelling</a> shows unaffordable housing and poor neighbourhoods both negatively affect mental health, reinforcing the need to provide both affordable and well-located housing.</p> <h2>The upcoming budget</h2> <p>While the <a href="https://www.dss.gov.au/sites/default/files/documents/05_2023/payments-cra_budget_fact_sheet_fa_0.pdf">15% increase</a> in the maximum rent assistance rate was welcomed in the last budget, the program is long overdue for a major restructure to target those in rental stress.</p> <p>Also, tax concessions on second properties should be wound back to reduce competition for those struggling to buy their first home. This would eventually help ease affordability pressures on low-income renters as more higher-income renters <a href="https://onlinelibrary.wiley.com/doi/abs/10.1111/1467-8454.12335">shift into homeownership</a>.</p> <p>The potential negative impacts on rental supply can be mitigated by careful design of tax and other changes that guard against market destabilisation concerns.</p> <p>Overall, housing affordability solutions have to be multi-faceted. The housing system is badly broken and meaningful repair cannot be achieved unless policymakers are willing to confront both supply and demand challenges.<!-- Below is The Conversation's page counter tag. Centenarian blood tests give hints of the secrets to longevity

<p><em><a href="https://theconversation.com/profiles/karin-modig-1473484">Karin Modig</a>, <a href="https://theconversation.com/institutions/karolinska-institutet-1250">Karolinska Institutet</a></em></p> <p>Centenarians, once considered rare, have become commonplace. Indeed, they are the <a href="https://www.weforum.org/agenda/2021/02/living-to-one-hundred-life-expectancy/">fastest-growing demographic group</a> of the world’s population, with numbers roughly doubling every ten years since the 1970s.</p> <p>How long humans can live, and what determines a long and healthy life, have been of interest for as long as we know. Plato and Aristotle discussed and <a href="https://pubmed.ncbi.nlm.nih.gov/12092789/">wrote about the ageing process</a> over 2,300 years ago.</p> <p>The pursuit of understanding the secrets behind exceptional longevity isn’t easy, however. It involves <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7105197/">unravelling the complex interplay</a> of genetic predisposition and lifestyle factors and how they interact throughout a person’s life. Now our recent study, <a href="https://link.springer.com/article/10.1007/s11357-023-00936-w">published in GeroScience</a>, has unveiled some common biomarkers, including levels of cholesterol and glucose, in people who live past 90.</p> <p>Nonagenarians and centenarians have long been of intense interest to scientists as they may help us understand how to live longer, and perhaps also how to age in better health. So far, studies of centenarians have often been small scale and focused on a selected group, for example, excluding centenarians who live in care homes.</p> <h2>Huge dataset</h2> <p>Ours is the largest study comparing biomarker profiles measured throughout life among exceptionally long-lived people and their shorter-lived peers to date.</p> <p>We compared the biomarker profiles of people who went on to live past the age of 100, and their shorter-lived peers, and investigated the link between the profiles and the chance of becoming a centenarian.</p> <p>Our research included data from 44,000 Swedes who underwent health assessments at ages 64-99 - they were a sample of <a href="https://pubmed.ncbi.nlm.nih.gov/28158674/">the so-called Amoris cohort</a>. These participants were then followed through Swedish register data for up to 35 years. Of these people, 1,224, or 2.7%, lived to be 100 years old. The vast majority (85%) of the centenarians were female.</p> <p>Twelve blood-based biomarkers related to inflammation, metabolism, liver and kidney function, as well as potential malnutrition and anaemia, were included. All of these <a href="https://www.nature.com/articles/s41591-019-0719-5">have been associated</a> with ageing or mortality in previous studies.</p> <p>The biomarker related to inflammation was uric acid – a waste product in the body caused by the digestion of certain foods. We also looked at markers linked to metabolic status and function including total cholesterol and glucose, and ones related to liver function, such as alanine aminotransferase (Alat), aspartate aminotransferase (Asat), albumin, gamma-glutamyl transferase (GGT), alkaline phosphatase (Alp) and lactate dehydrogenase (LD).</p> <p>We also looked at creatinine, which is linked to kidney function, and iron and total iron-binding capacity (TIBC), which is linked to anaemia. Finally, we also investigated albumin, a biomarker associated with nutrition.</p> <h2>Findings</h2> <p>We found that, on the whole, those who made it to their hundredth birthday tended to have lower levels of glucose, creatinine and uric acid from their sixties onwards. Although the median values didn’t differ significantly between centenarians and non-centenarians for most biomarkers, centenarians seldom displayed extremely high or low values.</p> <p>For example, very few of the centenarians had a glucose level above 6.5 earlier in life, or a creatinine level above 125.</p> <p>For many of the biomarkers, both centenarians and non-centenarians had values outside of the range considered normal in clinical guidelines. This is probably because these guidelines are set based on a younger and healthier population.</p> <p>When exploring which biomarkers were linked to the likelihood of reaching 100, we found that all but two (alat and albumin) of the 12 biomarkers showed a connection to the likelihood of turning 100. This was even after accounting for age, sex and disease burden.</p> <p>The people in the lowest out of five groups for levels of total cholesterol and iron had a lower chance of reaching 100 years as compared to those with higher levels. Meanwhile, people with higher levels of glucose, creatinine, uric acid and markers for liver function also decreased the chance of becoming a centenarian.</p> <p>In absolute terms, the differences were rather small for some of the biomarkers, while for others the differences were somewhat more substantial.</p> <p>For uric acid, for instance, the absolute difference was 2.5 percentage points. This means that people in the group with the lowest uric acid had a 4% chance of turning 100 while in the group with the highest uric acid levels only 1.5% made it to age 100.</p> <p>Even if the differences we discovered were overall rather small, they suggest a potential link between metabolic health, nutrition and exceptional longevity.</p> <p>The study, however, does not allow any conclusions about which lifestyle factors or genes are responsible for the biomarker values. However, it is reasonable to think that factors such as nutrition and alcohol intake play a role. Keeping track of your kidney and liver values, as well as glucose and uric acid as you get older, is probably not a bad idea.</p> <p>That said, chance probably plays a role at some point in reaching an exceptional age. But the fact that differences in biomarkers could be observed a long time before death suggests that genes and lifestyle may also play a role.<!-- Below is The Conversation's page counter tag. “The system is too soft”: Ally Langdon fires up over stabbing death of Vyleen White

<p>Ally Langdon has called for an overhaul of the youth crime system in the wake of the violent death of Queensland grandmother Vyleen White. </p> <p>The <em>A Current Affair</em> host was discussing the death of the 70-year-old, as she became visibly frustrated while talking about the rising rates of youth crime.</p> <p>Langdon hinted at a nationwide issue, citing the recent stabbing death of young doctor Ash Gordon in Melbourne, who was also allegedly murdered by a teenager less than a month ago.</p> <p>“Whatever we’re doing to deal with youth crime, it’s failing,” she said.</p> <p>“The police do everything they can, but the system is too soft on serious crime, and we have lost faith in it and our politicians.”</p> <blockquote class="instagram-media" style="background: #FFF; 8 reasons everyone should know their blood type

<p><strong>Why you should know your blood type</strong></p> <p>What’s in a blood type? Potentially a lot, according to research, including a review of studies published in the Wiley Interdisciplinary Reviews: Systems Biology and Medicine, that connects different blood groups to everything from risk of heart disease and dementia to urinary tract infections and the norovirus.</p> <p>While none of the studies are conclusive about cause and effect (they can’t say X blood type causes Y disease) and any increased risks are still pretty small, the research does highlight the importance of knowing your type – A, B, AB, or O – and how it could affect your wellbeing.</p> <p><strong>Blood clots: Type AB, A, and B increases risk</strong></p> <p>Danish researchers studied how blood type interacts with a genetic predisposition for deep-vein thrombosis (DVT), or blood clots in the lower legs that can travel to the lungs and become life-threatening. After analysing data on about 66,000 people over more than 30 years, they found that those with type AB, A, or B had a 40 per cent higher risk of DVT than people with type O, the most common type.</p> <p>When the scientists did further analysis to see which factors have the biggest impact on DVT risk on a population level, they found that an AB blood type contributed to about 20 per cent of blood clots; genetic mutations accounted for 11 per cent, being overweight accounted for 16 per cent, and smoking accounted for six per cent.</p> <p><strong>Heart disease: Type AB, B, and A all increase risk </strong></p> <p>People whose blood type is A, B, or AB have an increased risk of heart disease and shorter life spans than people who have type O blood, according to a large study published in BMC Medicine. After following more than 50,000 middle-age and elderly people for seven years, on average, researchers found that as many as nine per cent of cardiovascular deaths were attributed to having non-O blood types.</p> <p>However, as any doctor will tell you, lifestyle factors like weight, smoking and diet, which, unlike blood type, are modifiable, have a much greater impact on heart disease.</p> <p><strong>Stomach cancer: Types A and AB increases risk </strong></p> <p>Researchers have known for a while that people with blood type A are at risk for stomach cancer. But research published in BMC Cancer shows that people with blood type AB are also at risk. Using genetic data from a large number of cases and controls, researchers found a link between both blood types and gastric cancer in Chinese populations. A review of 39 previous studies confirmed their findings.</p> <p><strong>Fertility: Type O reduces it </strong></p> <p>Women with this blood type were twice as likely to have blood levels of the hormone FSH high enough to indicate low ovarian reserve, a measure of fertility, according to a study published in Human Reproduction. Researchers couldn’t say for sure why, though. Given that type O blood is the most prevalent, it doesn’t pay to worry too much about it. Age is a far more important risk factor for fertility problems.</p> <p><strong>Pregnancy risks </strong></p> <p>This has nothing to do with your “letter” blood type or the type determined by the ABO grouping system. This has to do with what’s known as the Rhesus (Rh) factor, which determines whether your blood type is positive or negative. This could cause complications in pregnant women if the baby’s Rh blood type is different from the mother’s.</p> <p>For instance, if the mother has a negative blood type and the baby has a positive one, the mother’s body can actually build up antibodies against the baby’s blood type. Luckily, this doesn’t affect the baby, but it could have a negative effect on future pregnancies. Fortunately, doctors can give pregnant women a shot early in their pregnancy that can prevent Rh-incompatibility problems.</p> <p><strong>Dementia and memory loss: Type AB increases risk </strong></p> <p>People with type AB blood have an 82 per cent greater risk for cognitive decline later in life, according to a study published in Neurology. That’s likely because they have larger amounts of what’s known as the Factor VIII protein, which helps with blood clotting.</p> <p>Study participants with higher levels of this protein were 24 per cent more likely to develop memory problems – regardless of their blood type – than people with lower levels. Blood type, however, is far from the only, or even most important, factor that affects your risk for cognitive decline.</p> <p><strong>Stroke: Type O has the lowest risk</strong></p> <p>People with a blood type other than O (the most common) have a higher risk of cardiovascular issues such as stroke, according to a study published in the Journal of Thrombosis and Haemostasis. Biologists are still investigating why this might be; one possible explanation is that non-O blood types contain more of the Von Willebrand factor, a protein that has been connected to blood clotting and stroke in the past.</p> <p><strong>Mosquitos like Type O blood </strong></p> <p>If you find yourself scratching bug bites all summer long, your blood type might be to blame. How does Australia’s health system stack up internationally? Not bad, if you’re willing to wait for it

<p><em><a href="https://theconversation.com/profiles/stephen-duckett-10730">Stephen Duckett</a>, <a href="https://theconversation.com/institutions/the-university-of-melbourne-722">The University of Melbourne</a></em></p> <p>When things are going bad in the health system, we are reassured we’ve got one of the best health systems in the world. But we’re rarely told where we actually stand relative to others.</p> <p>A <a href="https://www.oecd.org/health/health-at-a-glance/">new report</a> from the Organisation for Economic Cooperation and Development (OECD) shows where Australia is doing relatively well – and not so well. The report is more than 200 pages with dozens of charts and tables.</p> <p>Here we highlight five charts showing Australia’s relative performance. Overall, Australia’s health system performs well, but can come after long waits. And our use of antibiotics is trending in the wrong direction.</p> <h2>1. We spend less than average but live longer than average</h2> <p>Despite the rhetoric about the unsustainability of the health system, Australia performs well. When mapping health expenditure against life expectancy, Australia (marked by the red dot) sits in the best performing quadrant – and has done so for the past decade.</p> <p><iframe id="tc-infographic-996" class="tc-infographic" style="border: none;" src="https://cdn.theconversation.com/infographics/996/32f7548791bf05b7559d74976bfa0b955319adc5/site/index.html" width="100%" height="400px" frameborder="0"></iframe></p> <p>In contrast, the United States is stuck in the worst performing quadrant for the whole period – significantly higher spending than other countries with worse life expectancy.</p> <p>The life expectancy measure is used here but it involves an implicit assumption that the principal impact on life expectancy is from the health system, which is not really the case. Nevertheless, it is a good measure of overall system performance and combined with spending provides a good measure.</p> <h2>2. Most Australians rate their health as good or very good</h2> <p>The vast majority of Australians (about 85%) rate their health as good or very good, with Australia performing better on this metric than most other similar countries. Often good health is conflated with good health care, and the data show that Australia also has more doctors per head than other countries.</p> <p><iframe id="Ygo7Z" class="tc-infographic-datawrapper" style="border: none;" src="https://datawrapper.dwcdn.net/Ygo7Z/1/" width="100%" height="400px" frameborder="0"></iframe></p> <p>The founding charter of the World Health Organization (WHO) recognised that health is not just the absence of disease, but a “<a href="https://www.who.int/about/accountability/governance/constitution">state of complete physical, mental and social well-being</a>”. This points to a flaw in the nexus between good health and more health professionals. The WHO focus on well-being helps to explain why it is not surprising that, looking across countries, the number of doctors doesn’t appear to be a key determinant of performance on self-rated health.</p> <h2>3. It’s harder to get a bed in aged care</h2> <p>About 30% of people in OECD countries are over 65, while the Australian proportion is about 20%. The proportion of over-65s is rising everywhere.</p> <p>A minority of older Australians (14%) use aged care, with most of these using home care. However, monitoring access to residential aged care (represented here by the number of long-term care beds per thousand population over 65) might act as a “canary in the coal mine”, highlighting where access problems exist.</p> <p><iframe id="lBe4O" class="tc-infographic-datawrapper" style="border: none;" src="https://datawrapper.dwcdn.net/lBe4O/1/" width="100%" height="400px" frameborder="0"></iframe></p> <p>In Australia, access to aged care beds is falling, by about 27% between 2011 and 2021. We started in the middle of the pack so this is a concern and probably contributes to more Australians being stuck in acute hospitals, rather than being in more appropriate accommodation in residential aged care. This “<a href="https://www.ama.com.au/articles/hospital-exit-block-symptom-sick-system">exit block</a>” in turn leads to problems of ambulance ramping.</p> <h2>4. Australians wait too long for public hospital hip replacements</h2> <p>Most publicly funded health systems are characterised by long waiting times for access to planned procedures such as hip replacements. Some waiting is to be expected as part of efficient management of operating theatre scheduling. But long waits, especially when the person is in pain, reflect poorly on management of the public hospital system.</p> <p><iframe id="LNntD" class="tc-infographic-datawrapper" style="border: none;" src="https://datawrapper.dwcdn.net/LNntD/2/" width="100%" height="400px" frameborder="0"></iframe></p> <p>The data shows that almost two-thirds of people waiting for hip replacement surgery in Australia waited more than three months. This is marginally worse than the OECD average. Unfortunately, our performance is deteriorating.</p> <p>A number of states, such as <a href="https://www.health.vic.gov.au/planned-surgery-reform-blueprint">Victoria</a>, have developed strategies to improve the performance of the planned procedure system, or have identified opportunities for <a href="https://www.sahealth.sa.gov.au/wps/wcm/connect/public+content/sa+health+internet/about+us/about+sa+health/reporting+and+advisory+groups/health+performance+council/health+performance+council+reports">efficiency improvements in public hospitals</a> which would help address this issue.</p> <p>Although it’s understandable that <a href="https://www.sciencedirect.com/science/article/pii/S2049080122011554">planned procedures were affected</a> by the first few years of the COVID pandemic, governments should have adapted their funding and provision systems to bring waiting times back to the pre-pandemic levels.</p> <h2>5. Our use of antibiotics is going in the wrong direction</h2> <p>Antibiotics have saved millions of lives. But public health experts have long recognised the emerging <a href="https://www.who.int/news-room/fact-sheets/detail/antimicrobial-resistance">problem of antimicrobial resistance</a>, where inappropriate use of these drugs can lead to their reduced effectiveness over time.</p> <p>Worldwide campaigns to promote appropriate use of antibiotics are bearing fruit and across the OECD, use of antibiotics is going down.</p> <p>Unfortunately, Australia’s trend is in the reverse direction.</p> <p><iframe id="AK4GO" class="tc-infographic-datawrapper" style="border: none;" src="https://datawrapper.dwcdn.net/AK4GO/1/" width="100%" height="400px" frameborder="0"></iframe><!-- Below is The Conversation's page counter tag. "I've lost complete blood flow": Robert Irwin's near miss with python

<p>Young wildlife warrior Robert Irwin suffered a near miss during a rescue mission over the weekend, when he tried to relocate a carpet python off the road. </p> <p>The 19-year-old took to Instagram on Sunday to share a video of him almost getting bit by the wild snake. </p> <p>"Gee, that gets the heart rate up - he missed me by that much," he said when the snake struck at him. </p> <p>"He's grumpy... he's really keen on biting me... what a gorgeous snake, he's big, he's not venomous but... they're designed to constrict," he said as the python began wrapping its body around his arm to ''constrict" him. </p> <p>"He's got a good grip there, I've lost complete blood flow to my hand, it's completely blue.. and I have no feeling left in my hand," he added. </p> <p>He eventually managed to rescue the snake, and relocated it to a safe spot in the bush the day after. </p> <p>"Near miss! Worried about getting a blood test? 5 tips to make them easier (and still accurate)

Whether you’re getting a routine check-up, diagnosing a medical condition or monitoring treatment progress, understanding the process can make the experience more comfortable and effective.</p> <p>For the majority of patients, blood collections are a minor inconvenience. Others may feel <a href="https://www.sciencedirect.com/science/article/abs/pii/S0887618506000041">uneasy and anxious</a>.</p> <p>Preparation strategies can help get you through the procedure.</p> <h2>How blood is collected</h2> <p>During venipuncture (blood draw), the phlebotomist (blood collector) inserts a needle through the skin into a vein and a small amount of blood is collected and transferred into a test tube.</p> <p>Tubes are sent to a laboratory, where the blood is analysed. A laboratory technician may count or examine cells and measure the levels of minerals/salts, enzymes, proteins or other substances in the sample. For some tests, blood plasma is separated out by spinning (centrifuging) the sample. Others pass a light beam through the sample to determine the amount of a chemical present.</p> <p>For collection, the phlebotomist usually selects a vein in the crook of your elbow, where veins are readily accessible. Blood can also be drawn from veins in the wrists, fingers or heels. A tourniquet may be applied to restrict blood flow and make the chosen vein puff out.</p> <h2>Different tests require different preparation</h2> <p>Before a blood test, the GP or health-care provider will give you specific instructions.</p> <p>These may include fasting for up to 12 hours or temporarily discontinuing certain medications.</p> <p>It is crucial to follow these guidelines meticulously as they can significantly impact the accuracy of your test results. For example, fasting is required before glucose (blood sugar) and lipids (blood fats) testing because blood sugar and cholesterol levels typically increase after a meal.</p> <p>If the blood test requires fasting, you will be asked not to eat or drink (no tea, coffee, juice or alcohol) for about eight to 12 hours. Water is allowed but smoking should be avoided because it can increase <a href="https://diabetesjournals.org/care/article/19/2/112/19825/Acute-Effect-of-Cigarette-Smoking-on-Glucose">blood sugar, cholesterol and triglyceride levels</a>.</p> <p>Generally, you will be asked to fast overnight and have the blood collection done in the morning. Fasting for longer than 15 hours could impact your results, too, by causing dehydration or the release of certain chemicals in the blood.</p> <p>If you have diabetes, you must consult your doctor prior to fasting because it can increase the risk of hypoglycemia (low blood sugar) in people with type 1 diabetes. Most type 2 diabetics can safely fast before a blood test but there are some exceptions, such as people who are taking certain medications including insulin.</p> <h2>5 tips for a better blood test</h2> <p>To improve your blood collection experience, consider these tips:</p> <p><strong>1. Hydrate</strong></p> <p>Drink plenty of water right up to 30 minutes before your appointment. Adequate hydration improves blood flow, making your veins more accessible. Avoid <a href="https://academic.oup.com/labmed/article/34/10/736/2657269">strenuous exercise</a> before your blood test, which can increase some blood parameters (such as liver function) while decreasing others (such as sodium).</p> <p><strong>2. Loose clothing</strong></p> <p>Wear clothing that allows easy access to your arms to ensure a less stressful procedure.</p> <p><strong>3. Manage anxiety</strong></p> <p>If the sight of blood or the procedure makes you anxious, look away while the needle is inserted and try to keep breathing normally. Distraction can help – virtual reality has been <a href="https://pubmed.ncbi.nlm.nih.gov/31889358/">trialled</a> to reduce needle anxiety in children. You could try bringing something to read or music to listen to.</p> <p><strong>4. Know your risk of fainting</strong></p> <p>If you’re prone to fainting, make sure to inform the phlebotomist when you arrive. You can have your blood drawn while lying down to minimise the risk of passing out and injury. Hydration helps maintain blood pressure and can also <a href="https://www.ahajournals.org/doi/10.1161/01.CIR.0000101966.24899.CB">reduce the risk</a> of fainting.</p> <p><strong>5. Discuss difficult veins</strong></p> <p>Some people have smaller or scarred veins, often due to repeated punctures, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4989034/">chemotherapy</a> or blood thinner use. In such cases, venipuncture may require multiple attempts. It is important to talk to the phlebotomist if you feel discomfort or significant pain. A finger prick can be performed as an alternative for some tests, such as blood glucose levels. But other comprehensive tests require larger blood volume.</p> <h2>Blood draws after lymph node removal</h2> <p>Historically, there were concerns about drawing blood from an arm that had undergone lymph node removal. This was due to the risk of <a href="https://www.cancer.gov/about-cancer/treatment/side-effects/lymphedema/lymphedema-pdq#:%7E:text=Lymphedema%20is%20the%20build%2Dup,the%20way%20that%20it%20should.">lymphedema</a>, a condition marked by fluid build-up in the affected arm. Lymph nodes may have been removed (<a href="https://www.ncbi.nlm.nih.gov/books/NBK564397/#:%7E:text=Lymph%20node%20dissection%2C%20also%20known,surgical%20management%20of%20malignant%20tumors.">lymphadenectomy</a>) for cancer diagnosis or treatment.</p> <p>However, a <a href="https://ascopubs.org/doi/10.1200/JCO.2015.61.5948">2016 study</a> showed people who’ve had lymph nodes removed are not at a higher risk of developing lymphedema following blood draws, even when drawing blood from the affected arm.</p> <h2>After your blood test</h2> <p>The whole blood test procedure usually lasts no more than a few minutes. Afterwards, you may be asked to apply gentle pressure over a clean dressing to aid clotting and reduce swelling.</p> <p>If you do experience swelling, bruising or pain after a test, follow general first aid procedures to alleviate discomfort. These include applying ice to the site, resting the affected arm and, if needed, taking a pain killer.</p> <p>It is usually recommended you do not do heavy lifting for a few hours after a blood draw. Women and low-income earners miss out in a superannuation system most Australians think is unfair

<p><em><a href="https://theconversation.com/profiles/antonia-settle-1019551">Antonia Settle</a>, <a href="https://theconversation.com/institutions/the-university-of-melbourne-722">The University of Melbourne</a></em></p> <p>Most Australians think the superannuation system is unfair, with only one in three agreeing the retirement savings scheme is fair for most Australians, according to a survey conducted for the University of Melbourne.</p> <p>In fact, only about half of those <a href="https://melbourneinstitute.unimelb.edu.au/publications/research-insights/search/result?paper=4630688">surveyed</a> agreed superannuation works well for them.</p> <p>These results contradict a conventional view based on earlier studies and held by academics and many in the personal finance sector, that Australians give little thought to superannuation.</p> <p>A 2013 survey found Australians have <a href="https://search.informit.org/doi/abs/10.3316/INFORMIT.285049750322819">poor knowledge</a> of how the superannuation system works, while another study in 2022 highlighted <a href="https://melbourneinstitute.unimelb.edu.au/__data/assets/pdf_file/0011/4382057/HILDA_Statistical_Report_2022.pdf">low financial literacy</a> in general.</p> <p>Australians also showed <a href="https://behaviouraleconomics.pmc.gov.au/sites/default/files/projects/retirement-planning-saving-attitudes_0_0.pdf">little interest in superannuation</a>, according to a 2020 Department of Prime Minister and Cabinet survey, with few Australians showing interest in reading their superannuation statements, choosing their fund or making voluntary contributions.</p> <p>With Australian households seen as uninformed and uninterested, their opinions tend to be left out of the public debate. We hear much about the gender pension gap, for example, but little about what women actually think about superannuation.</p> <p>Similarly, the distribution of tax advantage in superannuation is hotly debated by economists but survey data tends to refrain from asking households what they think about equity in the superannuation system.</p> <p>The University of Melbourne survey of 1,003 Australians was undertaken by Roy Morgan Research in April.</p> <p>Its results show women and low-income households are widely seen as disadvantaged in the superannuation system.</p> <p>In fact, only one in five Australians see the superannuation system as well suited to the needs of women and of low-income households, while 70% believe super favours wealthy households.</p> <p><iframe id="5VX3K" class="tc-infographic-datawrapper" style="border: none;" src="https://datawrapper.dwcdn.net/5VX3K/1/" width="100%" height="400px" frameborder="0"></iframe></p> <p>This suggests although Australians may show little interest in the management of their super accounts and may report they find the system confusing or even <a href="https://www.professionalplanner.com.au/wp-content/uploads/2016/05/Attitudes-to-Super-Report-May-2016.pdf">boring</a>, they are surprisingly aware of how superannuation is distributed.</p> <h2>Women, singles and low-income earners miss out</h2> <p>The federal government’s 2020 <a href="https://treasury.gov.au/publication/p2020-100554">Retirement Income Review</a> documents these gaps. Renters, women, uncoupled households and those on low-incomes fare poorly in the retirement income system.</p> <p>With little super to supplement the public pension, these groups are vastly over-represented in elderly poverty statistics, which are among the <a href="https://www.oecd-ilibrary.org/sites/d76e4fad-en/index.html?itemId=/content/component/d76e4fad-en">highest in the OECD</a>.</p> <p>Mirroring the gaps in the superannuation system reported by the review, the University of Melbourne survey shows that it is outright homeowners and those who are married who believe the superannuation system works well.</p> <p>Concerns the system works poorly for women and low-income households are strongest among women and low-income households. Only one in three renters believe the superannuation system meets their needs.</p> <p><iframe id="N9GO6" class="tc-infographic-datawrapper" style="border: none;" src="https://datawrapper.dwcdn.net/N9GO6/1/" width="100%" height="400px" frameborder="0"></iframe></p> <p>This suggests individuals’ concerns about fairness in the superannuation system are driven by their own experiences of disadvantage, regardless of financial literacy.</p> <p>This is consistent with my own <a href="https://www.tandfonline.com/doi/full/10.1080/13563467.2023.2195159">research</a> into household attitudes to superannuation, which showed some resentment among women who were well aware their male partners had substantially higher superannuation balances than them.</p> <p>This all matters for policymakers.</p> <h2>Why public perceptions are important</h2> <p>In the short term, these results suggest public support for making super fairer is likely to be stronger than previously thought. Recent government changes to tax concessions on large balances, for example, could have gone much further without losing support from the 70% of households that think the system favours the wealthy.</p> <p>But it matters for the longer term too.</p> <p>Public perceptions of fairness, effectiveness and efficiency are crucial to policy sustainability. This is well established in the academic literature from <a href="https://onlinelibrary.wiley.com/doi/full/10.1111/spol.12683">B Ebbinghaus</a>, 2021 and <a href="https://onlinelibrary.wiley.com/doi/abs/10.1111/1911-3838.12171">H Chung et al.</a>, and accepted by the Retirement Income Review.</p> <p>The review assessed the public’s confidence in the system to both “deliver an adequate retirement income for them(selves) and (to) generate adequate outcomes across society”.</p> <p>As the review makes clear, the system must avoid a loss of public confidence from perceptions of unfairness.</p> <p>Yet perceptions of unfairness are exactly what the University of Melbourne results suggest. This would have been clearer to policymakers if they asked earlier.<!-- Below is The Conversation's page counter tag. The $500 million ATO fraud highlights flaws in the myGov ID system. Here’s how to keep your data safe

<p><em><a href="https://theconversation.com/profiles/rob-nicholls-91073">Rob Nicholls</a>, <a href="https://theconversation.com/institutions/unsw-sydney-1414">UNSW Sydney</a></em></p> <p>The Australian Tax Office (ATO) paid out more than half a billion dollars to cyber criminals between July 2021 and February 2023, according to an <a href="https://www.abc.net.au/news/2023-07-26/ato-reveals-cost-of-mygov-tax-identity-crime-fraud/102632572">ABC report</a>.</p> <p>Most of the payments were for small amounts (less than A$5,000) and were not flagged by the ATO’s own monitoring systems.</p> <p>The fraudsters exploited a weakness in the identification system used by the myGov online portal to redirect other people’s tax refunds to their own bank accounts.</p> <p>The good news is there’s plenty the federal government can do to crack down on this kind of fraud – and that you can do to keep your own payments secure.</p> <h2>How these scams work</h2> <p>Setting up a myGov account or a myGov ID requires proof of identity in the form of “<a href="https://www.afp.gov.au/sites/default/files/PDF/NPC-100PointChecklist-18042019.pdf">100 points of ID</a>”. It usually means either a passport and a driver’s licence or a driver’s licence, a Medicare card, and a bank statement.</p> <p>Once a myGov account is created, linking it to your tax records requires two of the following: an ATO assessment, bank account details, a payslip, a Centrelink payment, or a super account.</p> <p>These documents were precisely the ones targeted in three large data breaches in the past year: at <a href="https://theconversation.com/what-does-the-optus-data-breach-mean-for-you-and-how-can-you-protect-yourself-a-step-by-step-guide-191332">Optus</a>, at <a href="https://theconversation.com/medibank-hackers-are-now-releasing-stolen-data-on-the-dark-web-if-youre-affected-heres-what-you-need-to-know-194340">Medibank</a>, and at <a href="https://asic.gov.au/about-asic/news-centre/news-items/guidance-for-consumers-impacted-by-the-latitude-financial-services-data-breach/">Latitude Financial</a>.</p> <p>In this scam, the cyber criminal creates a fake myGov account using the stolen documents. If they can also get enough information to link to the ATO or your Tax File Number, they can then change bank account details to have your tax rebate paid to their account.</p> <p>It is a sadly simple scam.</p> <h2>How government can improve</h2> <p>One of the issues here is quite astounding. The ATO knows where salaries are paid, via the “<a href="https://www.ato.gov.au/business/single-touch-payroll/what-is-stp-/">single touch</a>” payroll system. This ensures salaries, tax and superannuation contributions are all paid at once.</p> <p>Most people who have received a tax refund will have provided bank account details where that payment can be made. Indeed, many people use precisely those bank account details to identify themselves to myGov.</p> <p>At present, those bank details can be changed within myGov without any further ado. If the ATO simply checked with the individual via another channel when bank account details are changed, this fraud could be prevented. It might be sensible to check with the individual’s employer as well.</p> <p>Part of the problem is the ATO has not been very transparent about the risks. If these risks were clearly set out, then calls for changes to ATO procedures would have been loud and clear from the cyber security community.</p> <p>The ATO is usually good at identifying when a cyber security incident may lead to fraud. For example, when the recruitment software company <a href="https://www.abc.net.au/news/2018-06-06/australian-data-may-be-compromised-in-pageup-security-breach/9840048?itm_campaign=newsapp">PageUp was hacked in 2018</a>, the ATO required people who may have been affected to reconfirm their identities. This was done without public commentary and represents sound practice.</p> <p>Sadly, the millions of records stolen in the Optus, Medibank and Latitude Financial breaches have not led to a similar level of vigilance.</p> <p>Another action the ATO could take would be to check when a single set of bank account details is associated with more than one myGov account.</p> <p>A national digital identity would also help. However, this system has been in development for years, is not universally popular, and may well be <a href="https://www.themandarin.com.au/226280-gallagher-warns-community-support-for-digital-identity-not-ubiquitous/">delayed</a> until after the federal election due in 2024.</p> <h2>Protecting yourself</h2> <p>The most important thing to do is make sure the ATO does not use a bank account number other than yours. As long as the ATO only has your bank account number to transfer your tax rebate, this scam does not work.</p> <p>It also helps to protect your Tax File Number. There are only four groups that ever need this number.</p> <p>The first is the ATO itself. The second is your employer. However, remember you do not need to give your TFN to a prospective employer, and your employer only needs your TFN <em>after</em> you have started work.</p> <p>Your super fund and your bank may ask for your TFN. However, providing your TFN to your super fund or bank is optional – it just makes things easier, as otherwise they will withhold tax which you will need to claim back later.</p> <p>Of course, all the usual data safety issues still apply. Don’t share your driver’s licence details without good reason. Take similar care with your passport. Your Medicare card is for health services and does not need to be shared widely.</p> <p>Don’t open emails from people you do not know. Never click links in messages unless you are sure they are safe. Most importantly, know your bank will not send you emails containing links, nor will the ATO.<!-- Below is The Conversation's page counter tag. Outrage after renters confronted with cleaning “scoring system”

<p dir="ltr">An Australian real estate agent has come under fire for their “demoralising” scoring system for tenants during routine inspections. </p> <p dir="ltr">One fired up renter said they were shocked to receive a scorecard from the agent after they complied with an inspection, and shared their feedback on Reddit. </p> <p dir="ltr">The scorecard rated the tenant’s efforts cleaning the property in 17 different rooms and areas, marking them on cleanliness, and if the areas were damaged or required maintenance. </p> <p dir="ltr">The renter was then also given an overall rating out of five stars, based on the upkeep of their rental home. </p> <p dir="ltr">The Reddit user showed that they received just three and half stars, despite being marked down in only one area due to a light globe not working.</p> <p dir="ltr">In their post, the tenant said they had made every effort to clean the home, adding that the rating was “demoralising”.</p> <p dir="ltr">“I’ve been renting a long time, and my deep cleaning routine is based around inspections,” they explained.</p> <p dir="ltr">“I go all out - every room in my little 3x2 villa is given a spring clean, every surface is washed including walls, doors, lights, windows, carpets etc.”</p> <p dir="ltr">“Had my latest inspection a few weeks ago and just received the report. Despite the agent commenting once again that she never sees houses as clean as mine, and taking 112 (!) photos of every single thing that shows how great the condition of the house is - having one light globe not working is so terrible that I’m scored three and a half out of five.”</p> <p dir="ltr">The renter went on to say that they had only recently discovered that the broken light globe was the real estate’s responsibility to replace, and does not warrant such a severe marking down in their “score”.</p> <p dir="ltr">“The globe in question is a downlight fitting. We have so many downlights that I don’t even use this particular one,” they continued.</p> <p dir="ltr">“I can’t replace the globe itself, I need to get into the roof to replace the whole unit. Thanks to people here I’ve learned this isn’t even my responsibility to fix!”</p> <p dir="ltr">The renter’s post sparked a huge outrage online, with many up in arms over the current state of the rental market and unrealistic expectations of estate agents. </p> <p dir="ltr">One person said, “I fully believe that the average renter keeps a home to a higher standard than an owner, and it seems deliberately demoralising that someone could be marked down for having a light out.”</p> <p dir="ltr">Another added, “This is ridiculously petty, and more importantly, totally irrelevant to the purpose of a periodic inspection. Which is to identify whether any maintenance is needed.”</p> <p dir="ltr">A third person took their anger out on the entire system, summing it up by writing, “I sort of wonder what little lightbulb clicks in your mind and you wake up one day and ask yourself, ‘How can I be a massive c*** who offers no value to society? An El Niño looms over Australia’s stressed electricity system – and we must plan for the worst

<p><a href="https://theconversation.com/profiles/dylan-mcconnell-1602">Dylan McConnell</a>, <em><a href="https://theconversation.com/institutions/unsw-sydney-1414">UNSW Sydney</a></em> and <a href="https://theconversation.com/profiles/iain-macgill-576">Iain MacGill</a>, <em><a href="https://theconversation.com/institutions/unsw-sydney-1414">UNSW Sydney</a></em></p> <p>The Bureau of Meteorology this week declared a 70% chance of an <a href="https://media.bom.gov.au/releases/1170/the-bureau-of-meteorology-issues-an-el-nino-alert/">El Niño</a> developing this year. It’s bad timing for the electricity sector, and means Australians may face supply disruptions and volatile prices.</p> <p><a href="http://www.bom.gov.au/climate/updates/articles/a008-el-nino-and-australia.shtml">El Niño events</a> are <a href="https://theconversation.com/from-floods-to-fire-a-climate-scientist-on-the-chances-el-nino-will-hit-australia-this-year-197408">associated with</a> increased temperatures and heatwaves. These conditions drive demand for electricity, especially in summer.</p> <p>These same conditions can also mean some generators don’t produce at full capacity. And unfortunately, the likely El Niño comes as the electricity sector grapples with other significant headwinds.</p> <p>Australia’s electricity grid may be fine this summer. And the hotter transmission lines get, the less electrical current they can safely carry. This lowers their capacity to transport energy.</p> <p>When the electricity grid is under stress, this can lead to “load shedding” or blackouts – when power companies deliberately switch off the power supply to groups of customers to prevent the overall system from becoming dangerously unstable.</p> <p>This happened in Victoria in early 2019, when more than <a href="https://www.abc.net.au/news/2019-01-26/victorian-blackouts-what-caused-them-and-is-this-the-new-normal/10751412">200,000 customers</a> lost power during a period of extreme heat.</p> <p>El Niño events are also associated with reduced rainfall. Among other effects on the electricity grid, this can reduce output from hydroelectricity generators (which produce electricity by pumping water through turbines). This <a href="https://theconversation.com/praying-for-rain-50085">occurred</a> in Tasmania in 2016, and contributed to an energy crisis in that state.</p> <h2>Other headwinds are blowing</h2> <p>Aside from facing a likely El Niño, the electricity sector faces other headaches.</p> <p>Earlier this year, the Australian Energy Market Operator <a href="https://aemo.com.au/newsroom/media-release/aemo-issues-nem-reliability-update">warned</a> electricity demand “may exceed supply” at times over the next decade due to factors such as weather conditions or generator outages.</p> <p>The market operator pointed to delays to the Snowy 2.0 hydro project and the gas-fired <a href="https://www.afr.com/companies/energy/new-snowy-ceo-can-t-rule-out-further-delays-at-projects-20230213-p5cjzr">Kurri Kurri Power Station</a>, both in New South Wales.</p> <p>The Kurri Kurri project has been delayed for a year. It was scheduled to begin operating in December this year – in time for the first summer since the Liddell coal-fired power station closed.</p> <p>The Australian Energy Market Operator said the electricity system was expected to meet the “reliability standard” in all regions for the next five years. The <a href="https://www.aemc.gov.au/energy-system/electricity/electricity-system/reliability#:%7E:text=What%20is%20the%20Reliability%20Standard,to%20be%20met%20each%20year.">standard requires</a> at least 99.998% of forecast demand be met each year. Unmet demand can lead to interrupted supply, or blackouts.</p> <p>But the operator also said delays to the Kurri Kurri project posed risks to reliability in NSW this summer.</p> <p>Adding to the pressures on the system, Queensland’s Callide C coal-fired power station is still not back to capacity more than two years after an explosion at the site. The station’s owners last week <a href="https://www.afr.com/companies/energy/energy-grid-fears-as-callide-c-return-delayed-until-mid-2024-20230529-p5dc90">announced</a> the plant would not be fully operational until mid-2024.</p> <p>Combine all this with a likely El Niño, and the electricity sector may be facing a challenging summer.</p> <h2>El Niño years are not normal</h2> <p>In August, the Australian Energy Market Operator is due to publish a new <a href="https://aemo.com.au/-/media/files/electricity/nem/planning_and_forecasting/nem_esoo/2022/esoo-and-reliability-forecast-methodology-document-2022.pdf">assessment</a> of the grid’s expected reliability over the next decade. It may well show reliability standards will be achieved.</p> <p>On first blush, that sounds like good news. However, the way the assessment is derived may mask the real risk during El Niño periods.</p> <p>The assessment combines a number of scenarios, which are based on different <a href="https://aemo.com.au/en/energy-systems/electricity/national-electricity-market-nem/nem-forecasting-and-planning/forecasting-and-reliability/projected-assessment-of-system-adequacy">forecasts</a> of electricity demand. The scenarios based on average weather conditions are given the most weight.</p> <p>But if an El Niño arrives, this summer will not be average. We’re likely to experience very hot and dry conditions. This may lead to higher demands on the energy system, and a greater likelihood of blackouts.</p> <p>This won’t be properly reflected in the assessment. But there are measures and technologies in place to help reduce the risks.</p> <p><a href="https://aemo.com.au/en/energy-systems/electricity/emergency-management/reliability-and-emergency-reserve-trader-rert">A mechanism</a> exists that allows the market operator to secure emergency energy reserves. It could mean, for example, calling on a large industrial plant to pause operations to reduce its electricity use, or starting up a standby diesel generator. The operator can start procuring this months ahead of time, and will no doubt be monitoring the situation closely.</p> <p>In the medium term, the uptake of so-called “consumer energy resources” such as rooftop solar farms and small-scale battery storage shows promise. These technologies are located at homes and businesses. They can reduce demand on the grid at peak times and can potentially be built faster than big projects.</p> <p>Longer term, we need to build more “stuff”. This includes renewable energy and other “dispatchable” resources – which can provide energy when it’s needed – as well as more transmission infrastructure.</p> <p>Several federal funding measures – the <a href="https://www.energy.gov.au/government-priorities/energy-supply/capacity-investment-scheme">Capacity Investment Scheme</a> and <a href="https://www.energy.gov.au/news-media/news/rewiring-nation-supports-its-first-two-transmission-projects">Rewiring the Nation</a> – might help realise these projects.</p> <p>The reality is that ageing coal plants are closing – and while they remain open, they’re contributing to reliability challenges in the energy system. Unchecked climate change will also add considerable strain, through natural disasters and more extreme weather.</p> <p>Unfortunately, investment in renewable and other low-emission technology has been <a href="https://www.cleanenergycouncil.org.au/news/clean-energy-construction-peaks-as-investment-pipeline-battles-headwinds">slower than necessary</a>. This has slowed Australia’s emissions reduction efforts and cast questions over the reliability of our energy supplies as an El Niño looms.<!-- Below is The Conversation's page counter tag. No, you can’t reverse ageing by injecting ‘young blood’ and fasting. But that doesn’t stop people trying

<p><a href="https://theconversation.com/profiles/rachael-jefferson-buchanan-297850">Rachael Jefferson-Buchanan</a>, <em><a href="https://theconversation.com/institutions/charles-sturt-university-849">Charles Sturt University</a></em></p> <p>Like many celebrities and entrepreneurs, 45-year-old US tech billionaire Bryan Johnson is <a href="https://www.smh.com.au/lifestyle/health-and-wellness/taking-the-blood-of-your-17-year-old-son-anti-ageing-has-gone-too-far-20230530-p5dcd6.html">trying to reverse the ageing process</a>.</p> <p>Spending an average of US$2 million a year on an anti-ageing regimen, Johnson <a href="https://medium.com/future-literacy/at-45-i-now-age-slower-than-the-average-10-year-old-6932448fc608">claims</a> he now ages slower than some children. He explains: “the pace my body accumulates ageing damage is less than the average ten year old”.</p> <p>Many of Johnson’s age-reversal methods are questionable, involve dodgy science, and have known side effects.</p> <p>While you can’t stop the ageing process, and the gradual decline our bodies experience as we advance in years, there are some things we can all do – for free – to maintain our health as we age.</p> <h2>What does Johnson do? And is it scientific?</h2> <p><strong>Fasting</strong></p> <p>Johnson reports fasting for 23 hours a day. He then eats <a href="https://medium.com/future-literacy/one-meal-23-hr-fast-100-nutrition-18187a2f5b">one meal a day</a>: 2,250 calories of nutrient-dense food “customised” to his body’s needs.</p> <p>Eating for time-restricted periods in the day can have a <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9650338/">positive effect</a> on how we <a href="https://pubmed.ncbi.nlm.nih.gov/29955217/">metabolise nutrients</a>, inflammation levels, hormonal regulation, and <a href="https://www.health.harvard.edu/blog/how-good-is-your-cardiometabolic-health-and-what-is-that-anyway-202208182803">cardiometabolic health</a> (blood sugar, <a href="https://www.mayoclinic.org/diseases-conditions/high-blood-cholesterol/in-depth/triglycerides/art-20048186">triglycerides</a>, cholesterol, blood pressure, BMI and waist circumference).</p> <p>However, a Spartan-like food intake can <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2121099/">impair</a> how our body responds to sugar (known as glucose tolerance). And it’s not necessarily any more effective for weight maintenance than <a href="https://pubmed.ncbi.nlm.nih.gov/29419624/">reducing calorie intake at each meal</a>.</p> <p>Large-scale, long-term human trials are needed to confirm the <a href="https://pubmed.ncbi.nlm.nih.gov/34728336/">limited risk-benefit</a> findings of fasting.</p> <p><strong>Acid peels</strong></p> <p>Johnson has weekly <a href="https://www.asds.net/skin-experts/skin-treatments/chemical-peels/chemical-peels-for-aging-skin">acid peels</a> (which use a mild acid to exfoliate the skin) to maintain a “youthful glow”.</p> <p>But you cannot smooth sagging facial skin or remove deep scars or wrinkles. Acid peels also <a href="https://www.mayoclinic.org/tests-procedures/chemical-peel/about/pac-20393473">come with risks</a>, including organ damage, infection, scarring and swelling.</p> <p><strong>Plasma infusions</strong></p> <p>Perhaps the most bizarre youth-inducing procedure Johnson has attempted is receiving blood transfusions from his 17-year-old son.</p> <p>US biotech companies have explored <a href="https://www.theguardian.com/society/2020/feb/02/could-young-blood-stop-us-getting-old-transfusions-experiments-mice-plasma">plasma infusions</a> to tackle age-related diseases in humans for decades. But there are no proven clinical benefits.</p> <p><a href="https://www.redcrossblood.org/donate-blood/blood-donation-process/what-happens-to-donated-blood/blood-transfusions/risks-complications.html">Side effects from blood transfusions</a> include blood-borne infections, fever and allergic reactions.</p> <h2>Historical attempts to stay youthful</h2> <p>Humans have been experimenting with <a href="https://academic.oup.com/biomedgerontology/article/59/6/B515/662071">anti-ageing methods for centuries</a>. These have included all sorts of behavioural and lifestyle practices that are quirky, questionable, and even sadistic.</p> <p>Ancient practices included <a href="https://www.marieclaire.com/beauty/news/a14382/anti-aging-beauty-through-history/">crocodile dung face masks</a>, which the Greeks and Romans used to brighten their complexions.</p> <p>Romans also used <a href="https://beautytap.com/2019/03/donkey-milk">donkey milk</a> and <a href="https://www.ancient-origins.net/history/swans-fat-crocodile-dung-and-ashes-snails-achieving-beauty-ancient-rome-003240">swan fat</a> to minimise wrinkles, due to their acclaimed rejuvenating properties.</p> <p><a href="https://www.livescience.com/44071-cleopatra-biography.html">Cleopatra</a> apparently took daily baths in sour donkey milk. To sustain this lavish habit, she had a <a href="https://www.naturanecosmetics.com/en/content/26-faits-historiques">herd of 700 donkeys</a>. Sour milk contains <a href="https://science.jrank.org/pages/3780/Lactic-Acid-Lactic-acid-in-foods.html">lactic acid</a>, a naturally occurring <a href="https://www.mecca.com.au/edits/ingredients/alpha-hydroxy-acids/">alpha-hydroxy acid (AHA)</a> that exists in many modern-day exfoliants. Exposure to cold is a firm favourite.</p> <p><a href="https://www.nature.com/articles/s43587-023-00383-4">Some research</a> suggests this could have <a href="https://neurosciencenews.com/cold-aging-22928/">benefits</a> relating to longevity, by slowing cellular degeneration, <a href="https://www.cryo.com.au/anti-ageing-benefits-of-whole-body-cryotherapy/">stimulating collagen and elastin production</a>, increasing the metabolism, and reducing inflammation.</p> <p>Dutch motivational speaker Wim Hof includes <a href="https://www.wimhofmethod.com/cold-water-immersion">cold water immersion</a> as one of the three pillars of his <a href="https://www.wimhofmethod.com/">Wim Hof Method</a> to “increase mind-body connection”.</p> <p>Athletes such as <a href="https://www.dailymail.co.uk/sport/football/article-2469985/Cristiano-Ronaldo-buys-Cryotherapy-chamber.html">Cristiano Ronaldo</a> use <a href="https://my.clevelandclinic.org/health/treatments/21099-cryotherapy">cryotherapy</a>, exposing their bodies to extremely cold temperatures for two to four minutes to decrease the signs of ageing and enhance their general health.</p> <p>However, the <a href="https://www.medicinenet.com/what_are_the_side_effects_of_cryotherapy/article.htm">risks of cryotherapy</a> include bone fractures, frostbite, nerve damage, bleeding, cramping, swelling and skin infections.</p> <h2>So what can we do to age well?</h2> <p>Two of the more mainstream anti-ageing methods that Johnson recommends are the daily self-care habits of sleep and exercise.</p> <p>He has a <a href="https://medium.com/future-literacy/sleep-and-impulse-control-87e844218ff2">strict sleep schedule</a> that involves retiring to bed at 8pm, with a one-hour wind-down in a darkened room.</p> <p>Adults report poorer sleep quality and difficulty being able to sleep for long enough <a href="https://www.news-medical.net/health/How-Does-the-Suprachiasmatic-Nucleus-(SCN)-Control-Circadian-Rhythm.aspx">as they age</a>. He does a “<a href="https://www.youtube.com/watch?v=HNywRJgCRaQ">Blueprint</a>” workout that includes specially designed daily techniques, as well as <a href="https://www.hsph.harvard.edu/nutritionsource/high-intensity-interval-training/">high-intensity interval training sessions</a>, hiking and playing sport.</p> <p>From middle age onwards, we all need to exercise regularly, to increase our muscle mass, bone density, strength, endurance, coordination and balance. One of the greatest health risks for older people is <a href="https://www.ncbi.nlm.nih.gov/books/NBK560761/">falling</a>, which balance, flexibility, endurance and strength training <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC381224/">can help</a> reduce. Physical activity can bring <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5408452/">social benefits</a> in older adults if undertaken in groups, and there are well-known <a href="https://www.whiddon.com.au/yourlife/the-mental-health-benefits-of-exercise-for-older-adults/">mental health gains</a>.</p> <p>Small changes in sleep, diet (eating <a href="https://health.gov/news/202107/nutrition-we-age-healthy-eating-dietary-guidelines">plenty of vegetables, fruit, wholegrains, healthy fats, and enough protein</a>), and exercise can support <a href="https://bmcgeriatr.biomedcentral.com/articles/10.1186/s12877-020-01900-5">healthy ageing</a>, reducing the chance of early death, and helping us all to lead an active and independent life in our senior years. Want an easy $400 a year? Ditch the gas heater in your home for an electric split system

<p>Earlier this month, regulators <a href="https://theconversation.com/why-are-electricity-prices-going-up-again-and-will-it-ever-end-201869">flagged</a> power price rises in Queensland, New South Wales, Victoria and South Australia. Like many people, you’re probably wondering how you can minimise the financial pain.</p> <p>Getting rid of gas and electrifying everything in your home can save you money. <a href="https://renew.org.au/wp-content/uploads/2022/11/Report-Limiting-energy-bills-by-getting-off-gas.pdf">Modelling</a> by not-for-profit organisation Renew showed annual bills last year for a <a href="https://www.nathers.gov.au/owners-and-builders/home-energy-star-ratings">seven-star</a> all-electric home with solar power were between 69% (Western Sydney) and 83% (Hobart) cheaper than bills for a three-star home with gas appliances and no solar. </p> <p>There are other reasons to kick the gas habit, too. As renewables form an ever-growing part of Australia’s energy mix, electrifying the home increasingly helps tackle climate change. What’s more, there are sound <a href="https://www.unsw.edu.au/news/2023/02/a-heated-debate--how-safe-are-gas-stoves--">health</a> reasons to get rid of gas appliances.</p> <p>But where do you start? And how do you get the best bang for your buck? Here, I offer a few tips.</p> <h2>A quick guide to home energy use</h2> <p>Australian home energy use can be separated into a few categories: </p> <ul> <li>space heating and cooling</li> <li>water heating</li> <li>cooking</li> <li>vehicles </li> <li>other appliances (many of which are largely already electric). </li> </ul> <p>Of the appliances that typically depend on gas, the <a href="https://www.rewiringaustralia.org/report/castles-and-cars-discussion-paper">largest component</a> (37%) is space heating, followed by hot water (24%) and cooking (6%). </p> <p>This varies between states. Victoria, for example, is particularly dependent on gas. </p> <p>But the breakdown above gives some insight into the largest contributors to energy costs in the average Australian home – particularly in the cooler southern regions.</p> <p>While both gas and electricity costs are rising, as they are now in most states, all-electric homes can expect lower overall increases. <a href="https://renew.org.au/wp-content/uploads/2022/11/Report-Limiting-energy-bills-by-getting-off-gas.pdf">Analysis</a> by Renew has shown ditching the old gas heater in favour of a split system/reverse cycle air-conditioner (without solar panels) can lead to average savings of A$546 each year in Canberra, $440 in Adelaide, $409 in Melbourne and $256 in Perth. </p> <p>Heating a space with a reverse-cycle air conditioner is about four times more efficient than using natural gas. And when the electricity is generated by renewables, it can be done with zero greenhouse gas emissions.</p> <p>And what about heating water? Using the existing electricity grid, the cost of using an electric heat pump is around half that of using a natural gas water heater. </p> <p>The costs fall even lower if a household shifts to solar panels subsidised or financed by government, backed by a home battery providing the energy. In this case, heating costs are around a third of using gas.</p> <h2>So what’s the payback?</h2> <p>Buying new appliances costs money. So it’s important to examine the “payback” period - in other words, the length of time it takes for energy bill savings to equal the cost of the initial investment in a new appliance. </p> <p>The payback period can vary depending on:</p> <ul> <li>cost and quality of the appliance</li> <li>an appliance’s energy rating</li> <li>size of the system</li> <li>for space heating, whether a split system is replacing an existing ducted system or being added on externally.</li> </ul> <p>A <a href="https://www.climatecouncil.org.au/wp-content/uploads/2022/10/CC_MVSA0323-CC-Report-Gas-vs-Electricity_V4-Single.pdf">report</a> last year by the Climate Council calculated the approximate cost differences between higher and lower-end electric appliances. Lower-end hot water heat pumps, reverse-cycle air conditioner and induction stoves were priced around $7,818 all together, while higher-end appliances cost around $14,936 together.</p> <p>Both scenarios included installation costs and $3,000 for electrical upgrades and other costs. </p> <p>The payback period for low-priced appliances ranged from five years in Hobart and Canberra to 15 years in Perth and Sydney. Higher-priced appliances were in the order of eight to ten years for most cities and 12, 16 and 19 years for Melbourne, Perth and Sydney respectively.</p> <p>Rolled out at scale, household electrification is also a feasible way to reduce gas demand. It may be the only practical option available to decarbonise residential energy. </p> <p>As research <a href="https://www.sciencedirect.com/science/article/pii/S0196890422013802#b0130">recently suggested</a>, so-called “green” hydrogen – made by using low-carbon electricity to split water into hydrogen and oxygen – is unlikely to emerge as a cheap replacement for gas boilers. And why look for a technological solution to a problem we already know how to solve?</p> <p>Modelling by <a href="https://environmentvictoria.org.au/wp-content/uploads/2020/06/Vic-Gas-Market-Demand-Side-Study-Final-Report-1.pdf">Environment Victoria</a> has shown installing heat pumps for heating and cooling would reduce statewide gas use by 48 petajoules a year, compared to the relatively minimal 0.5 petajoules saved by installing induction cooktops. </p> <p>At the same scale – and using a similar technology – replacing gas hot water with heat pump hot water reduces household gas use by 10 petajoules each year. That’s an enormous saving of gas.</p> <h2>The bigger picture for all-electric homes</h2> <p>Existing homes can benefit from a combination of electrification, rooftop solar and batteries. Major overhaul of Aussie superannuation system touted

<p>There may be a major change coming to Australia's $3.3 trillion superannuation system, with Treasurer Jim Chalmers looking to crack down on early access.</p> <p>Chalmers will be speaking to call for an objective for superannuation that will emphasise the need for it to be preserved until retirement.</p> <p>This would make it harder for Australians to access their funds early.</p> <p>The federal opposition are taking a different stance and have pushed for early access to remain open as part of a first home buyers scheme.</p> <p>John Kehoe, Australian Financial Review economics editor, told Today $36 billion had been withdrawn from Australians’ collective super during the COVID-19 pandemic.</p> <p>"That's something that Labor wants to shut the door on," he said.</p> <p>Kehoe said there were two sides to the first home buyers scheme, saying that people accessing their super early could drive prices higher.</p> <p>"The retirement income system showed the best way to have financial security in retirement is owning your own home," he said.</p> <p>"It is people renting in retirement that do it really tough.”</p> <p>Independent Senator Jacqui Lambie expressed her view on the proposal, saying life wasn't "black and white.”</p> <p>"There are things that happen in our lives where that money may come in handy, whether it is part of that money, or 20 per cent of that money, just to keep us afloat," she said.</p> <p>"And especially in the next two years, if we are going into recession, if there are guys out there who can dig in to make sure we can keep the roof over their head, to continue to pay their house rates, we have to be a bit more flexible than that when we are going through tough times."</p> <p>Image credit: Getty</p>

"You have to beg for help": how our welfare system pressures people to perform vulnerability

<p>People who rely on welfare payments to survive are often required to repeatedly tell stories of their personal hardships.</p> <p>In a <a href="https://www.theguardian.com/australia-news/2022/jun/07/job-seekers-could-have-welfare-stopped-under-onerous-new-points-based-system-advocates-warn">conditional welfare system</a>, many must regularly attend compulsory appointments, job search training courses, and self-development and treatment programs simply to receive their payments.</p> <p>People in extreme hardship often tell their stories even more frequently as they seek extra relief from non-government charities and community providers.</p> <p>Those on income support payments below the relative poverty line feel the crunch of <a href="https://www.ncoss.org.au/wp-content/uploads/2022/07/NCOSS_CostOfLiving22_FINAL_DESIGNED.pdf">inflation and rising living costs</a> most severely. This means many will require extra support from welfare services to meet their basic needs.</p> <p>Integral to this system is the idea of “performing vulnerability”.</p> <p>“Performing vulnerability” – a term I borrow from UK-based researcher <a href="https://policy.bristoluniversitypress.co.uk/vulnerability-and-young-people">Kate Brown</a> to update Australian academic <a href="https://www.cambridge.org/core/books/lowest-rung/FA159318C2D046EDD3C9347C8B8E4F2E">Mark Peel’s</a> idea of “performing poverty” – is not just about repeatedly describing personal hardship. </p> <p>It points to the expectation to describe hardship in particular ways that are recognisable – and hence believable – to support providers.</p> <p>My book, <a href="https://policy.bristoluniversitypress.co.uk/making-a-life-on-mean-welfare">Making a Life on Mean Welfare: Voices from Multicultural Sydney </a>shows how the expectation to perform vulnerability to access support shapes experiences on both sides of the welfare frontline. </p> <p>It can compound the cycle of disadvantage associated with receiving welfare in the long term. It does so by <a href="https://journals.sagepub.com/doi/10.1177/0038026119876775">fostering mistrust</a> between welfare users and providers, as well as tainting how people in need of support see themselves and their situation.</p> <h2>‘Tell me your story’</h2> <p>For my doctoral research, I spent 18 months speaking to welfare users and workers in culturally and linguistically diverse southwest Sydney. I also observed different aspects of service delivery while volunteering at a community welfare organisation. I interviewed 25 welfare users and 11 community welfare practitioners.</p> <p>As a researcher of everyday experiences of welfare and poverty, I know all too well what it is like to ask people to tell their stories of hardship yet again.</p> <p>I also grew up in an impoverished family reliant on welfare to get by. I know firsthand what the impact of retelling stories of hardship can be, particularly when the audience is, as Peel <a href="https://www.cambridge.org/core/books/lowest-rung/FA159318C2D046EDD3C9347C8B8E4F2E">puts</a> it, “someone who has the power to give or deny them something they need”.</p> <p>One of the community welfare practitioners I interviewed summed it up by saying: "They’re coming again feeling ashamed. They’ve knocked on someone’s door, to tell yet again how shitty their situation is."</p> <p>Her response was to chat and put them at ease before saying, “Can you tell me your story?” She would follow up by saying, “You’ve given me some insight, let’s formalise your story a little bit.”</p> <p>Some welfare workers showed more scepticism, particularly when it came to giving out emergency relief. </p> <p>When someone refused to share more than the minimum information required to be eligible for extra assistance, one welfare worker commented: "That person doesn’t want to take responsibility."</p> <p>Another practitioner told me, “That woman dramatised her situation,” but quickly added, “That doesn’t mean she wasn’t genuine.”</p> <p>The willingness of people seeking assistance to disclose personal hardships and do so convincingly impacts on how deserving they may come across to those delivering support. The pressure to perform can overshadow encounters between welfare users and workers even when it doesn’t determine the outcome.</p> <h2>‘It’s your dignity’</h2> <p>Among the most marginal welfare recipients I spoke to, “performing vulnerability” was another toll of poverty.</p> <p>Those experiencing the worst hardship frequently told me about having to explain “the ins and outs” and feeling “embarrassed”, “intimidated” or “uncomfortable” when they had to present to welfare agencies.</p> <p>Two young people (whom I have given fictional names) powerfully conveyed the cost of telling all about their struggles:</p> <blockquote> <p>Kane: Often if you go to them sorts of people (welfare agencies) you’ve gotta put it all out there, that you’re homeless, that you got nothing, you got no friends, no family – and then they’re gonna go boom “alright” (you get the help you came for)…</p> <p>Nessa: Yeah, that’s what I had to do to get a house and it’s embarrassing (talking over each other) I think it’s embarrassing.</p> <p>Kane: You gotta go down to those levels you know – it’s wrong.</p> <p>Nessa: When you gotta expose everything and don’t want to, it’s, like, your dignity.</p> <p>Kane: Yeah, it’s everything.</p> </blockquote> <h2>Performing is not pretending</h2> <p>The most marginal welfare users get a great deal of practice performing their hardship. But knowing how to tell their story a certain way is not the same as pretending.</p> <p>Not only do people at the sharp end of the welfare system have to endure the hardships of poverty, but they must then recite it in a way that registers as genuine, pressing and beyond reprieve.</p> <p>As a woman living on the disability support pension put it:"You don’t have the flexibility that a rich person has to respond to crisis, so you have to beg for help. That takes time!
