Parents whose newborn died at Northern Beaches Hospital speak out

<p>The grieving parents of a newborn who died following a series of medical delays at Sydney’s Northern Beaches Hospital say their daughter’s death was “completely avoidable” – and are calling for the facility to be returned to full public control.</p> <p>Harper Atkinson died in February, a day after experiencing severe complications at birth at the 488-bed hospital, which operates under a controversial public-private partnership model. Her parents, Leah Pitman and Dustin Atkinson, believe she could have been saved if urgent care had been available when it was needed most.</p> <p>“She should be here. She should be in our arms,” Leah <a href="https://www.abc.net.au/news/2025-04-16/harper-atkinson-death-northern-beaches-hospital-parents-speak/105180276" target="_blank" rel="noopener">told ABC’s 7.30</a>. “But she’s not – and it’s because there was no urgency. No theatre ready. No proper staffing. And now we’ve lost her.”</p> <p>Harper’s death is the second tragedy in recent months to put Northern Beaches Hospital under scrutiny. In September 2024, toddler Joe Massa collapsed and later died after a three-hour wait in the hospital’s emergency department. Healthscope, the private operator of the facility, admitted that was an “unacceptable failing”.</p> <p>The NSW government has since vowed never to repeat the public-private model used at the hospital, which serves around 350,000 people on Sydney’s northern beaches.</p> <p>Leah was initially sent home from the hospital on a Friday due to staff shortages. She returned the next evening after going into labour at home. Though her midwife had prepared a bath for a water birth, things quickly went wrong.</p> <p>“I felt this intense pain in my lower back, and then my waters broke – with blood,” Leah recalled. “I looked down and just thought, ‘Oh God, is that normal?’”</p> <p>By 9:30pm, concern was mounting. But it wasn’t until around 10:30pm that an obstetrician arrived and diagnosed a suspected placental abruption – a life-threatening condition for both mother and baby. He immediately called for a Category One caesarean section, the most urgent level of surgical intervention.</p> <p>But despite the emergency, Leah was left waiting.</p> <p>“They kept saying, ‘Theatre’s not ready. Theatre’s not ready,’” she said. “I assumed it was busy. We later found out there is no 24/7 theatre.”</p> <p>Northern Beaches Hospital operates its operating theatre using an on-call model on weekends. Staff must be within 30 minutes’ reach – a delay Leah and Dustin believe cost their daughter her life.</p> <p>“I was on the operating table 33 minutes after the call,” Leah said. “Harper wasn’t delivered until nearly an hour after the emergency was declared.”</p> <p>When she was born at 11:52pm, Harper was unresponsive. It took 21 agonising minutes before she drew her first breath.</p> <p>The following day, Leah and Dustin were told Harper would not survive. They made the heartbreaking decision to turn off her life support.</p> <p>“I should be holding her for the rest of my life, not just once,” Leah said through tears. “Once isn’t enough.”</p> <p>The family says they were told during a hospital debrief that it was not “economically feasible” to run a 24/7 theatre service. For Dustin, that explanation only deepened their pain.</p> <p>“Prioritising profit over healthcare. That’s what went wrong,” he said.</p> <p>Northern Beaches Hospital has launched a review into Harper’s death. But her parents say the issue is already painfully clear. “Time delays,” Leah said. “Things weren’t urgent enough.”</p> <p>The couple is now calling for the hospital to be brought fully back under public control – and for systemic change in how emergency obstetric care is delivered across the state.</p> <p>“I feel incredibly angry,” Leah said. “Her death, we feel, was completely avoidable.”</p> <p>“We’re sharing this because Harper deserves to be more than just another sad story,” she added. “We want change – so this never happens to another family.”</p> <p><em>Image: YouTube / ABC</em></p>

"He was just a genius": Shock after former AFL star dies at just 42

<p>Former AFL player Andrew Krakouer has passed away at the age of 42, prompting an outpouring of grief from former teammates, coaches and the broader football community.</p> <p>Krakouer, who played for Richmond and Collingwood, reportedly suffered a suspected heart attack at a family home in Perth on Sunday. His sudden passing has left many in shock, especially those who played alongside him during his impressive AFL career.</p> <p>Having played 102 games for the Tigers, Krakouer made a remarkable return to elite football after a personal setback, going on to play 35 matches for the Magpies. He was a key player in Collingwood’s 2011 grand final against Geelong, where he kicked three goals.</p> <p>Before his Collingwood stint, Krakouer dominated in the WAFL, winning the Sandover Medal in 2010 and delivering a best-on-ground performance in Swan Districts' premiership victory. His 42-disposal, four-goal effort in that grand final remains one of the greatest individual displays in WAFL history.</p> <p>Bouncing back from time in prison, Krakouer was recruited by Collingwood ahead of the 2011 season. He made an instant impact, scoring 35 goals across 23 games and winning Mark of the Year for his spectacular high-flying grab. However, his career was hampered by an ACL injury in early 2012. Despite this setback, he made a strong return later that year, featuring in three finals and kicking four goals in the qualifying final against Port Adelaide.</p> <p>Following a limited 2013 season, he was delisted but continued to be involved in football through media roles. He appeared on the <em>Marngrook Footy Show</em> and later co-hosted <em>Yokayi Footy</em>, also contributing as a commentator on both TV and radio.</p> <p>Tributes have poured in from former teammates and coaches across the country, commemorating Krakouer’s talent and character.</p> <p>“Rest in peace Andrew Krakouer. A beautiful man who was loved by all his teammates. Always greeted you with a hug and a smile. Love to Jimmy and Andrew’s daughters and all his family. Very sad day,” said Richmond great Matthew Richardson.</p> <p>Fellow ex-Tiger and former club CEO Brendon Gale also expressed his sorrow. “So saddened by the tragic and unexpected passing of Andrew Krakouer. Krak was a special talent — but more than that, he was a deeply loved husband, father, son, teammate, and friend. My condolences are with the Krakouer family and all who loved him. Rest easy Krak.”</p> <p>Collingwood’s former coach Nathan Buckley was visibly shaken upon hearing the news while live on radio. “That’s really sad news. That’s tragic, so young at 42,” Buckley said on SEN. “A really calm, quiet teammate, who was respectful and went about his business. I really enjoyed the years with him, that’s really sad for his family and his mates.”</p> <p>Buckley also paid tribute to Krakouer’s footballing prowess, particularly his ability to produce spectacular moments on the field. “We’d get him in whenever we could,” Buckley recalled. “He was just a genius. He was just a player who could do the special things. The hanger he took in 2011 (to win Mark of the Year) was elite.</p> <p>“He just loved the game, found a way to impact, and could do things that other players couldn’t do. He was exceptional.”</p> <p>Krakouer came from a legendary footballing family. Beloved champion athlete dies from rare brain tumour

<p>A beloved Australian athlete has tragically died after a high-profile battle with a rare and aggressive brain tumour. </p> <p>Jenny Alcorn, a champion triathlete, passed away on Monday at the age of 66, with her partner Margot Rupe breaking the devastating news. </p> <p>“She pushed every boundary in life and it was incredible,” Rupe said. “She was the love of my life for 20 years, she helped me raise two amazing children."</p> <p>“Yes, triathlon was her life but she always enjoyed all the sports our kids did, she never missed a TSS First 15 game, she never missed a Bond Uni rugby game."</p> <p>“Even on Saturday afternoon, she sat up and watched our son play rugby in Japan, so she was a pretty amazing person."</p> <p>“We’re all heartbroken, our souls are heartbroken, she did everything with love, she loved her family, she loved me, she loved our kids and embraced everything she did.”</p> <p>Alcorn was a sporting legend and Alcorn was recognised just last year at the AusTriathlon Awards, as she was particularly well known in Queensland as a coach, a competitor, and an inspiration to many. </p> <p>In 2016, at the age of 55, Alcorn came out of retirement and won the famous Kona Ironman World Championships in her age group.</p> <p>Last December, Alcorn’s “three-decade contribution” to Australia’s triathlon scene was recognised with the President’s Special Recognition Award, as the Trizone website said, “Her honour highlighted not just her achievements as an athlete and coach, but her inspiring resilience while battling a rare brain tumour."</p> <p>Surfers Paradise Triathlon Club member Dan Teasdale and close friend of Alcorn's paid tribute to his late friend. </p> <p>“She was always a fantastic spectator so you’d hear her yelling out ‘go Danny boy’ on the side of the race,” he said.</p> <p>“She was a very caring, loving person but a tough-as-nails athlete. Rock star's fiancée dies after falling overboard on cruise

<p>The fiancée of famous rockstar Taime Downe has died after falling overboard on an 80s music-themed cruise. </p> <p>The family of Kimberly Burch announced her death in a emotional post on Facebook, writing, "It is with broken hearts that we share the sad news that Kimberly Burch has passed away."</p> <p>"She was a beloved Daughter, Sister, Sister-in-law and Aunt. She will be greatly missed by all that knew and loved her."</p> <p>"We ask for prayers during this time for our family as we try to wrap our minds around this heartbreaking tragedy. We love and miss you Kimberly Burch!!"</p> <p>Burch, who worked as a realtor, joined the cruise on Sunday, as her partner's band Faster Pussycat was booked as one of the musical acts on board the seven-day cruise.</p> <p>By Monday, Burch had gone overboard roughly 30kms from the Bahamas and was presumed dead, with her family sharing their message confirming her death on social media on Tuesday. Her body remains missing.</p> <p>The couple shared a loved-up photo of them on the cruise less than 24 hours before her untimely death. </p> <p>Downe's band Faster Pussycat, an '80s hard rock and glam metal group known for their hits including <em>House Of Pain</em>, had two performances scheduled over the cruise – one on Sunday and another for Tuesday night.</p> <p>The cruise, billed as an ideal trip for lovers of '80s music, is set to return to port on Sunday March 9th.</p> <p>In a statement to <em><a title="Variety" href="https://variety.com/2025/music/news/kimberly-burch-dies-80s-cruise-faster-pussycat-1236327187/" target="_blank" rel="noopener">Variety</a></em>, Royal Caribbean said: "Our crew immediately launched a search and rescue effort, is working with local authorities, and we are providing support and assistance to the guest's family during this difficult time."</p> <p>"To respect the privacy of our guest's family, we have no additional details to share."</p> <p><em>Image credits: Instagram </em></p>

Dolly Parton's husband dies at age 82

<p>Dolly Parton's husband, Carl Dean, has died at the age of 82. </p> <p>Dean was notoriously private during their almost 60 years of marriage, after he tied the knot with the country music icon in 1966. </p> <p>Parton announced the sad news of her loss in a statement on her Instagram, saying "words can't do justice" to the "love [they] shared for over 60 years".</p> <p>"Carl Dean, husband of Dolly Parton, passed away March 3rd in Nashville at the age of 82," the statement read.</p> <p>"He will be laid to rest in a private ceremony with immediate family attending. He was survived by his siblings Sandra and Donnie."</p> <p>"Carl and I spent many wonderful years together. Words can't do justice to the love we shared for over 60 years. <p>James Harrison OAM, the famous Australian blood donor who saved the lives of 2.4 million babies by donating his rare plasma, has died aged 88. </p> <p>Australian Red Cross Lifeblood has confirmed that Harrison died "peacefully" on February 17 at Peninsula Village Nursing Home on the NSW Central Coast. </p> <p>Harrison, also known as the Man with the Golden Arm, began donating in the 1950s became the world's most prolific blood and plasma donor thanks to his rare antibody, Anti-D, which helps mothers who are at risk of passing on deadly antibodies to their babies. </p> <p>Since he started donating at the age of 18, Harrison has donated 1173 times, continuing his kind act up until his retirement in 2018 aged 81. </p> <p>His family have since paid tribute to him, with his daughter Tracey Mellowship, remembering him as a generous soul. </p> <p>"James was a humanitarian at heart, but also very funny," she said. </p> <p>"In his last years, he was immensely proud to become a great grandfather to two beautiful grandchildren, Trey and Addison.</p> <p>"As an Anti-D recipient myself, he has left behind a family that may not have existed without his precious donations."</p> <p>Lifeblood Chief Executive Officer Stephen Cornelissen added that Harrison's impact has made a difference all around the world. </p> <p>"James was a remarkable, stoically kind, and generous person who was committed to a lifetime of giving and he captured the hearts of many people around the world," Cornelissen said.</p> <p>"It was James' belief that his donations were no more important than any other donors', and that everyone can be special in the same way that he was.</p> <p>"James extended his arm to help others and babies he would never know a remarkable 1173 times and expected nothing in return."</p> <p>"He leaves behind an incredible legacy, and it was his hope that one day, someone in Australia would beat his donation record," he added. </p> <p>"On behalf of Lifeblood, and the entire Australian community, we thank James for the incredible life- saving contribution he made and the millions of lives he saved."</p> <p>Harrison's rare blood and plasma may continue to save lives even after his death, with researches hoping they would be able to use his blood to develop lab-grown Anti-D. </p> <p><em>Image: Nine News</em></p>

Hollywood star dies aged 39

<p>Michelle Trachtenberg has passed away aged 39. </p> <p>The former child star rose to fame in the 1996 film <em>Harriet the Spy</em>, before going on to have memorable roles on popular TV shows including <em>Buffy the Vampire Slayer</em> and <em>Gossip Girl</em>. </p> <p>Police responded to a  911 call on Wednesday just after 8am local time, where “officers observed a 39-year-old female unconscious and unresponsive,” inside a Manhattan apartment, according to a NYPD statement. </p> <p>Paramedics pronounced her dead at the scene, with no foul play suspected. </p> <p>Her official cause of death will come from the New York Medical Examiner. </p> <p>A NYPD source old <em>ABC News</em> that Trachtenberg may have been experiencing complications from a recent liver transplant, and is believed to have died of natural causes. </p> <p>"The family requests privacy for their loss. There are no further details at this time," a representative for Trachtenberg told <em>CNN </em>in a statement.</p> <p>Trachtenberg appeared in 66 episodes of <em>Buffy the Vampire Slayer</em> playing Dawn Summers, the younger sister of the title character played by Sarah Michelle Gellar. </p> <p>She also featured in 28 episodes of <em>Gossip Girl </em>as Georgina Sparks, between 2008 and 2012. </p> <p><em>Gossip Girl </em>creators Josh Schwartz and Stephanie Savage have paid tribute to the actress, writing:  “We are deeply saddened by the news of Michelle’s passing. Michelle was so funny, kind, and talented." </p> <p>“Her portrayal of Georgina Sparks was an iconic fan favorite, and grew from a one season villain to a beloved character who returned over the course of six seasons. Princess Diana's former lover opens up in rare interview

<p>Army Major James Hewitt, who had a five-year affair with the late Princess Diana from 1986, has spoken about the royal in a rare television interview with <em>Good Morning Britain </em>on Tuesday. </p> <p>When asked about whether he thinks Diana may have been able to reunite her sons, who have barely spoken in five years, he replied: "I think any mother would be worried and concerned about such a rift, as you put it." </p> <p>"And she'd do her best to try and get them together."</p> <p>Major Hewitt, who now spends his time doing aid work in Ukraine, revealed that the last time he had spoken to the late Princess was  "just after" her <em>BBC Panorama interview </em>with Martin Bashir in November 1995, during which she admitted to their affair.</p> <p>He described the conversation as  "distant" and admitted that the interview did "create problems" for him from which he was "trying to move on".</p> <p>After the affair was revealed, Major Hewitt was labelled as a "love rat" by the tabloid media and found his reputation in ruins. </p> <p>He also said that the interview was "very distressing" for the late Princess, as someone she had trusted had used their connection to make money.</p> <p>"It was a stitch-up job," he said of the <em>Panorama</em> interview. </p> <p> "It was appalling of Bashir to [have] inveigled, lied to her, criminal activity, absolutely appalling."</p> <p>"As I've said, I hate bullies and bullying and arrogant people and I think it's appalling."</p> <p>Major Hewitt, who was Diana's riding instructor at the time, has previously denied claiming to have been a "father figure" to the princes but has said that he "played with them, swam with them, taught them to ride".</p> <p>The Duke of Sussex had also addressed the rumours that Major Hewitt was his biological father in his memoir <em>Spare</em>, saying that he felt the rumours were designed to turn him into a "laughing stock".</p> <p>"One cause of the rumour was Major Hewitt's red hair, but another was sadism," he wrote in his memoir. </p> <p><em>Image: Ken McKay/ITV/ Shutterstock Editorial</em></p> <p> </p>

Legendary soul singer dies aged 88

<p>Roberta Flack, the legendary Grammy-winning singer and pianist, has passed away at the age of 88. Known for her intimate vocal style and timeless hits such as "Killing Me Softly With His Song" and "The First Time Ever I Saw Your Face", Flack was one of the most influential recording artists of the 1970s and continued to impact music for decades.</p> <p>Flack died at home surrounded by her family, her publicist Elaine Schock confirmed in a statement on Monday (early Tuesday AEDT). She had been battling ALS, also known as Lou Gehrig’s disease, since 2022, which had rendered her unable to sing.</p> <p>Flack’s rise to fame came unexpectedly in her early 30s when Clint Eastwood featured "The First Time Ever I Saw Your Face" in his 1971 film <em>Play Misty for Me</em>. The song, a hushed and emotional ballad, captivated audiences, topped the Billboard pop charts in 1972, and earned a Grammy for Record of the Year. She repeated this achievement the following year with "Killing Me Softly With His Song", making history as the first artist to win consecutive Record of the Year Grammys.</p> <p>Discovered in the late 1960s by jazz musician Les McCann, Flack’s voice was praised for its profound emotional depth. McCann famously remarked, "Her voice touched, tapped, trapped, and kicked every emotion I’ve ever known." Unlike some of her contemporaries, Flack often chose a reflective and measured approach in her performances, blending elements of jazz, soul\ and classical music. Her music resonated deeply with fans and also aligned with the social and civil rights movements of the era. Beauty pageant queen dies after horrific car crash

<p>American beauty pageant queen Kadance Fredericksen has died at the age of 18 after a horrific head-on car crash. </p> <p>According to multiple US outlets, Fredericksen, who was named Miss Okaloosa County Teen USA, tragically died on February 17 following a head-on collision with a tractor-trailer while driving on a Florida highway.</p> <p>The sudden death of the Miss Teen USA title holder was confirmed in a heartbreaking post on The Miss Florida USA Instagram account, saying "our pageant community has lost a beautiful soul" and noting she "was a true light in this world".</p> <p>The Miss Florida USA organisation wrote in their Instagram post underneath a black and white shot of the pageant queen: "It is with profound sadness and a heavy heart that we share the passing of one of our beloved delegates, Kadance Fredericksen, Miss Okaloosa County."</p> <p>"Our pageant community has lost a beautiful soul – one who was a true light in this world. Infant tragically dies at Melbourne family daycare

<p>A tragic incident has unfolded at a family daycare centre in Melbourne, where an infant has passed away. The devastating event has left the local community in shock as authorities work to determine the circumstances surrounding the child's death.</p> <p>Authorities responded to the facility in Skye, an hour southeast of the CBD, following reports of the baby’s death. Witnesses described a chaotic and distressing scene as first responders worked frantically to save the child. Authorities are interviewing witnesses and reviewing any available security footage to piece together the events leading up to the tragedy.</p> <p>Investigators continue to examine the circumstances surrounding the tragedy as the community awaits further details. "Profound sadness": Bali bombing hero dies at age 67

<p>Bali bombings survivor and hero Erik de Haart has died at the age of 67. </p> <p>The Sydney man was in Bali in 2002 with his Coogee Dolphins rugby league team when terrorists detonated bombs in two busy bars, killing 202 people, including 88 Australians.</p> <p>Mr De Haart was dubbed a hero after he was credited with pulling people out of the wreckage.</p> <p>He died aged 67 died of a suspected heart attack on Sunday.</p> <p>De Haart was a lifelong member of the Coogee Dolphins, with club president Wayne Hack issuing a statement after his tragic passing. </p> <p>“It is with profound sadness that we advise of the sudden passing of Coogee Dolphins life member Erik de Haart,” he said. “Erik was a loyal servant, life member and sponsor of our club."</p> <p>“He will forever be remembered in the immediate aftermath of the Bali tragedy of 2002 as a hero. Erik continued his ongoing support of the Coogee Dolphins for many years and in 2005 was honoured with a life membership."</p> <p>“The Coogee Dolphins would like to extend our deepest and heartfelt sympathies to Erik’s immediate and extended family and his many friends.”</p> <p>During the devastating 2002 attack in Bali, de Haart lost six of his friends but still helped save others by pulling them from the burning rubble. </p> <p>"I heard these girls crying for help, screaming. They were behind a section of the roof that had fallen in," de Haart told AAP in 2016.</p> <p>"The flames from the burning thatch were probably knee to mid-thigh high. I had to make a decision. Do I try and get them out and perhaps die in the attempt?"</p> <p>"I had to walk away and leave those girls to die and it's something that's haunted me for a long, long time ...
