What does a good death look like when you’re really old and ready to go?

<div class="theconversation-article-body"> <p><em><a href="https://theconversation.com/profiles/naomi-richards-182120">Naomi Richards</a>, <a href="https://theconversation.com/institutions/university-of-glasgow-1269">University of Glasgow</a></em></p> <p><a href="https://www.huffingtonpost.co.uk/entry/hawaii-legalizes-assisted-suicide_us_5ac6c6f5e4b0337ad1e621fb">Hawaii</a> recently joined the growing number of states and countries where doctor-assisted dying is legal. In these jurisdictions, help to die is rarely extended to those who don’t have a terminal illness. Yet, increasingly, very old people, without a terminal illness, who feel that they have lived too long, are arguing that they also have a right to such assistance.</p> <p>Media coverage of <a href="https://www.washingtonpost.com/news/to-your-health/wp/2018/05/09/this-104-year-old-plans-to-die-tomorrow-and-hopes-to-change-views-on-assisted-suicide/?utm_term=.b00a9036f9bc">David Goodall</a>, the 104-year-old Australian scientist who travelled to Switzerland for assisted dying, demonstrates the level of public interest in ethical dilemmas at the <a href="https://www.bmj.com/content/361/bmj.k1891.full">extremities of life</a>. Goodall wanted to die because he no longer enjoyed life. Shortly before his death, he told reporters that he spends most of his day just sitting. “What’s the use of that?” he asked.</p> <p><a href="https://www.sciencedirect.com/science/article/pii/S0277953615002889?via%3Dihub">Research</a> shows that life can be a constant struggle for the very old, with social connections hard to sustain and health increasingly fragile. <a href="https://www.ncbi.nlm.nih.gov/pubmed/25982088">Studies</a> looking specifically at the motivation for assisted dying among the very old show that many feel a deep sense of loneliness, tiredness, an inability to express their individuality by taking part in activities that are important to them, and a hatred of dependency.</p> <p>Of the jurisdictions where assisted dying is legal, some make suffering the determinant (Canada, for example). Others require a prognosis of six months (California, for example). Mainly, though, the focus is on people who have a terminal illness because it is seen as less of an ethical problem to hasten the death of someone who is already dying than someone who is simply tired of life.</p> <h2>Why give precedence to physical suffering?</h2> <p>Assisted dying for people with psychological or existential reasons for wanting to end their life is unlikely to be supported by doctors because it is not objectively verifiable and also potentially remediable. In the Netherlands, despite the legal power to offer assistance where there is no life-limiting illness, doctors are <a href="https://www.ncbi.nlm.nih.gov/pubmed/25693947">seldom convinced</a> of the unbearable nature of non-physical suffering, and so will rarely administer a lethal dose in such cases.</p> <p>Although doctors may look to a physical diagnosis to give them confidence in their decision to hasten a patient’s death, physical symptoms are often not mentioned by the people they are assisting. Instead, the most common reason given by those who have received help to die is <a href="https://www.nejm.org/doi/full/10.1056/NEJMms1700606">loss of autonomy</a>. Other common reasons are to avoid burdening others and not being able to enjoy one’s life – the exact same reason given by Goodall. This suggests that requests from people with terminal illness, and from those who are just very old and ready to go, are not as different as both the law – and doctors’ interpretation of the law – claim them to be.</p> <h2>Sympathetic coverage</h2> <p>It seems that the general public does not draw a clear distinction either. Most of the media coverage of Goodall’s journey to Switzerland was sympathetic, to the dismay of <a href="http://www.carenotkilling.org.uk/press-releases/centenarians-assisted-suicide/">opponents</a> of assisted dying.</p> <p>Media reports about <a href="https://www.theguardian.com/tv-and-radio/2014/sep/02/mary-berry-great-british-bake-off-centenarian-assisted-dying">ageing celebrities</a> endorsing assisted dying in cases of both terminal illness and very old age, blur the distinction still further.</p> <p>One of the reasons for this categorical confusion is that, at root, this debate is about what a good death looks like, and this doesn’t rely on prognosis; it relies on <a href="https://www.tandfonline.com/doi/full/10.1080/01459740.2016.1255610">personality</a>. And, it’s worth remembering, the personalities of the very old are as diverse as those of the very young.</p> <p>Discussion of assisted suicide often focuses on concerns that some older people may be exposed to coercion by carers or family members. But older people also play another role in this debate. They make up the rank and file <a href="http://www.ep.liu.se/ej/ijal/2012/v7/i1/a01/ijal12v7i1a01.pdf">activists</a> of the global right-to-die movement. In this conflict of rights, protectionist impulses conflict with these older activists’ demands to die on their own terms and at a time of their own choosing.</p> <p>In light of the <a href="https://www.nih.gov/news-events/news-releases/worlds-older-population-grows-dramatically">unprecedented ageing</a> of the world’s population and increasing longevity, it is important to think about what a good death looks like in deep old age. In an era when more jurisdictions are passing laws to permit doctor-assisted dying, the choreographed death of a 104-year-old, who died listening to Ode to Joy after enjoying a last fish supper, starts to look like a socially approved good death.<!-- Below is The Conversation's page counter tag. An unbroken night’s sleep is a myth. Here’s what good sleep looks like

They’re in good company – many Australians hold <a href="https://doi.org/10.1016/j.jpsychores.2014.09.011">the same belief</a>.</p> <p>In reality, healthy sleep is cyclic across the night, as you move in and out of the different stages of sleep, often waking up several times. Some people remember one or more of these awakenings, others do not. Let’s consider what a healthy night’s sleep looks like.</p> <h2>Sleep cycles are a roller-coaster</h2> <p>As an adult, our sleep moves through <a href="https://www.sleepfoundation.org/stages-of-sleep">different cycles</a> and brief awakenings during the night. Sleep cycles last roughly 90 minutes each.</p> <p>We typically start the night with lighter sleep, before moving into deeper sleep stages, and rising again into rapid eye movement (REM) sleep – the stage of sleep often linked to vivid dreaming.</p> <p>If sleeping well, we get most of our deep sleep in the first half of the night, with <a href="https://www.nhlbi.nih.gov/health/sleep/stages-of-sleep#:%7E:text=During%20REM%20sleep%2C%20your%20eyes,from%20acting%20out%20your%20dreams">REM sleep</a> more common in the second half of the night.</p> <p>Adults usually move through five or six sleep cycles in a night, and it is entirely <a href="https://doi.org/10.1016/j.jpsychores.2014.09.011">normal</a> to wake up briefly at the end of each one. That means we might be waking up five times during the night. This can increase with older age and still be healthy. If you’re not remembering these awakenings that’s OK – they can be quite brief.</p> <h2>What does getting a ‘good’ sleep actually mean?</h2> <p>You’ll often hear that adults <a href="https://www.sleephealthfoundation.org.au/sleep-topics/how-much-sleep-do-you-really-need#:%7E:text=Sleep%20requirements%20stabilise%20in%20early,their%20best%20the%20next%20day">need between seven and nine hours</a> of sleep per night. But good sleep is about more than the number of hours – it’s also about the quality.</p> <p>For most people, sleeping well means being able to fall asleep soon after getting into bed (within around 30 minutes), sleeping without waking up for long periods, and waking feeling rested and ready for the day.</p> <p>You shouldn’t be feeling excessively sleepy during the day, especially if you’re regularly getting at least seven hours of refreshing sleep a night (this is a rough rule of thumb).</p> <p>But are you noticing you’re feeling physically tired, needing to nap regularly and still not feeling refreshed? It may be worthwhile touching base with your general practitioner, as there a range of possible reasons.</p> <h2>Common issues</h2> <p>Sleep disorders are common. Up to <a href="https://pubmed.ncbi.nlm.nih.gov/35478719/">25% of adults</a> have insomnia, a sleep disorder where it may be hard to fall or stay asleep, or you may wake earlier in the morning than you’d like.</p> <p>Rates of common sleep disorders such as insomnia and <a href="https://www.mayoclinic.org/diseases-conditions/sleep-apnea/symptoms-causes/syc-20377631">sleep apnoea</a> – where your breathing can partially or completely stop many times during the night – also increase with age, affecting <a href="https://pubmed.ncbi.nlm.nih.gov/32280974/">20% of early adults</a> and 40% of people in <a href="https://pubmed.ncbi.nlm.nih.gov/35082023/">middle age</a>. There are effective treatments, so asking for help is important.</p> <p>Beyond sleep disorders, our sleep can also be disrupted by chronic health conditions – such as <a href="https://www.sciencedirect.com/science/article/abs/pii/S0012369218311139">pain</a> – and by <a href="https://onlinelibrary.wiley.com/doi/10.1111/jsr.13075">certain medications</a>.</p> <p>There can also be other reasons we’re not sleeping well. Some of us are woken by children, pets or traffic noise during the night. These “<a href="https://www.tandfonline.com/doi/full/10.1080/07420528.2021.2003375">forced awakenings</a>” mean we may find it harder to get up in the morning, take longer to leave bed and feel less satisfied with our sleep. For some people, night awakenings may have no clear cause.</p> <p>A good way to tell if these awakenings are a problem for you is by thinking about how they affect you. When they cause feelings of frustration or worry, or are impacting how we feel and function during the day, it might be a sign to seek some help.</p> <p>We also may struggle to get up in the morning. This could be for a range of reasons, including not sleeping long enough, going to bed or waking up at irregular times – or even your own <a href="https://www.sleephealthfoundation.org.au/sleep-categories/circadian-rhythm-disorders#:%7E:text=The%20circadian%20rhythms%20throughout%20the,regulation%20of%20our%20sleeping%20patterns">internal clock</a>, which can influence the time your body prefers to sleep.</p> <p>If you’re regularly struggling to get up for work or family needs, it can be an indication you may need to seek help. Some of these factors can be explored with a sleep psychologist if they are causing concern.</p> <h2>Can my smart watch help?</h2> <p>It is important to remember sleep-tracking devices can <a href="https://www.nature.com/articles/s41746-024-01016-9">vary in accuracy</a> for looking at the different sleep stages. While they can give a rough estimate, they are not a perfect measure.</p> <p>In-laboratory <a href="https://pubmed.ncbi.nlm.nih.gov/31277862/">polysomnography</a>, or PSG, is the best standard measure to examine your sleep stages. A PSG examines breathing, oxygen saturation, brain waves and heart rate during sleep.</p> <p>Rather than closely examining nightly data (including sleep stages) from a sleep tracker, it may be more helpful to look at the patterns of your sleep (bed and wake times) over time.</p> <p><a href="https://jcsm.aasm.org/doi/10.5664/jcsm.8356">Understanding your sleep patterns</a> may help identify and adjust behaviours that negatively impact your sleep, such as your bedtime routine and sleeping environment.</p> <p>And if you find viewing your sleep data is making you feel worried about your sleep, this may not be useful for you. Most importantly, if you are concerned it is important to discuss it with your GP who can refer you to the appropriate specialist sleep health provider.<!-- Below is The Conversation's page counter tag. Is there anything good about menopause? Yep, here are 4 things to look forward to

<div class="theconversation-article-body"><em><a href="https://theconversation.com/profiles/yvonne-middlewick-1395795">Yvonne Middlewick</a>, <a href="https://theconversation.com/institutions/edith-cowan-university-720">Edith Cowan University</a></em></p> <p>Menopause is having a bit of <a href="https://journals.sagepub.com/doi/full/10.1177/13675494231159562">a moment</a>, with less stigma and more awareness about the changes it can bring.</p> <p>A recent senate inquiry <a href="https://www.aph.gov.au/Parliamentary_Business/Committees/Senate/Community_Affairs/Menopause/Report">recommended</a> public education about perimenopause and menopause, more affordable treatments and flexible work arrangements.</p> <p>But like many things in life the experiences of menopause are on a continuum. While some women find it challenging and require support, others experience some physical and emotional benefits. These are rarely reported – but we can learn from the research available and, importantly, from people’s lived experiences.</p> <p>Here are four changes to look forward to once you reach menopause.</p> <h2>1. No more periods or related issues</h2> <p><a href="https://theconversation.com/what-are-the-most-common-symptoms-of-menopause-and-which-can-hormone-therapy-treat-225174">Menopause</a> is considered “complete” 12 months after the final period of a woman (or person assigned female at birth) who previously menstruated.</p> <p>Perhaps unsurprisingly, the benefit at the top of the list is no more periods (unless you are taking hormone therapy and still have your womb). This can be particularly beneficial for women who have had to manage <a href="https://www.mdpi.com/1660-4601/18/4/1951">erratic, unpredictable and heavy bleeding</a>.</p> <p>At last, you don’t need to keep sanitary protection in every bag “just in case”. No more planning where the bathroom is or having to take extra clothes. And you’ll save money by not purchasing sanitary products.</p> <p>There is also good news for women who have had heavy bleeding due to uterine fibroids – common benign gynaecological tumours that affect up to 80% of women. The evidence <a href="https://journals.lww.com/menopausejournal/abstract/2020/02000/uterine_fibroids_in_menopause_and_perimenopause.17.aspx">suggests</a> hormonal changes (for women not taking hormone therapy) can lead to a reduction in the size of fibroids and relieve symptoms.</p> <p>Women who suffer from menstrual migraine may experience an improvement in migraines post-menopause as their hormonal fluctuations begin to settle – but the timeframe for this <a href="https://www.maturitas.org/article/S0378-5122(20)30329-7/abstract">remains unclear</a>.</p> <p>For some women, no more periods also means more participation in social activities from which they may have been excluded due to periods. For example, <a href="https://www.tandfonline.com/doi/full/10.1080/08952841.2014.954502">religious activities or food preparation</a> in some cultures.</p> <h2>2. Getting your body and your groove back</h2> <p>Throughout their reproductive lives, women in heterosexual relationships are usually the ones expected to be <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6115298/">proactive about preventing pregnancy</a>.</p> <p>Some post-menopausal women <a href="https://www.tandfonline.com/doi/full/10.1080/08952841.2018.1396783">describe</a> a re-emergence of their sexuality and a sense of sexual freedom that they had not previously experienced (despite contraceptive availability) as there is no longer a risk of pregnancy.</p> <p>A participant in my research <a href="https://eprints.soton.ac.uk/451254/">into women’s experiences of menopause</a> described the joy of no longer being child-bearing age:</p> <blockquote> <p>I’ve got a body back for me, you know, coz I can’t get pregnant, not that I haven’t enjoyed having [children] and things like that and it was a decision to get pregnant but I feel like, ooh my body isn’t for anybody now but me, people, you know?</p> </blockquote> <p>For women who have chosen to be child-free there may also be a sense of freedom from social expectations. People will likely stop asking them when they are planning to have children.</p> <h2>3. A new chapter and a time to focus on yourself</h2> <p>Another participant <a href="https://eprints.soton.ac.uk/451254/">described</a> menopause as an unexpected “acceleration point” for change.</p> <p>Women told us they were more accepting of themselves and their needs rather than being focused on the needs of other people. Researchers have previously <a href="https://sk.sagepub.com/books/individualization">tracked this shift</a> from “living for others” to “a life of one’s own”.</p> <p>Some women find the strength of emotions at this time a challenge, whereas others find their potency can <a href="https://doi.org/10.1145/3290605.3300710">facilitate liberation</a> – enabling them to speak their minds or be more assertive than at any other time in their lives.</p> <h2>4. Increased self-confidence</h2> <p>A new sense of liberation can fuel increased self-confidence at menopause. This has been <a href="https://doi.org/10.1145/3290605.3300710">reported</a> in studies based on in-depth <a href="https://doi.org/10.1080/0167482X.2016.1270937">interviews with women</a>.</p> <p>Confidence boosts can coincide with <a href="https://eprints.soton.ac.uk/451254/">changes in career and sometimes in relationships</a> as priorities and self-advocacy transform.</p> <h2>Life on the other side</h2> <p>It can be hard to think about what is good about menopause, particularly if you are having challenges during perimenopause – but these can get better with time.</p> <p>In <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6910086/">cultures where women are valued as they become older</a>, women describe themselves as positively contributing to the community. They find they gain power and respect as they age.</p> <p>We need to work towards more positive societal attitudes on this front. Our bodies change across the lifespan and are remarkable at every stage, including menopause.<!-- Below is The Conversation's page counter tag. First look at Rebel Wilson's Italian wedding

<p>Rebel Wilson has <a href="https://oversixty.com.au/lifestyle/relationships/rebel-wilson-ties-the-knot-in-italian-destination-wedding" target="_blank" rel="noopener">tied the knot</a> with her partner Ramona Agruma, with the Aussie actress sharing the first glimpse into their special day. </p> <p>Sharing a picture of herself and new wife, who donned matching white wedding dresses by designer Pronovias, Wilson captioned the snap: "Married" alongside a ring emoji. </p> <p>She added their wedding date, writing: "Sardinia 28.9.24."</p> <p>The happy couple smiled widely as they looked loved-up while holding their bouquets, with the stunning Italian water behind them. </p> <p>Wilson's bridal gown featured an off-the-shoulder design and a full, A-line skirt, while she had her blonde hair pinned back on one side with a jewelled hair clip. </p> <p>Fans and followers flocked to comment underneath, including Wilson's former Pitch Perfect co-star, Adam Devine, who wrote, "Both of you look stunning! Congrats Rebs. Look at us all grown up! What does family look like in Australia? It’s more diverse than you think

It’s the version of society our policies and systems are built around.</p> <p>But Australia’s families are far more diverse. Some are multigenerational, some are sole-parent, some are blended. We need to understand how families have evolved over time and what that means for the social fabric of our country.</p> <p>Our <a href="https://www.uniting.org/content/dam/uniting/documents/families-report/uniting-families-report-2024.pdf">new research</a>, released today, charts years of data to map out what families have looked like historically and what they look like now. We also looked at how these families function, including income, wealth, labour division and care responsibilities.</p> <h2>Charting change over time</h2> <p>Families are the primary social environment in which we are cared for. As such, they play a fundamental role in our development and in making a thriving society.</p> <p>Of the <a href="https://www.abs.gov.au/media-centre/media-releases/2021-census-shows-changing-face-australias-6-million-families">six million</a> children and young people, aged 0–19 years, currently growing up in Australia, the majority learn and develop with the care and support of families. More than two and a half million families are raising children under the age of 15.</p> <p>Our understanding of who counts as family has expanded enormously over the past 50 years. But too often, we assume families are nuclear. Research too is guilty of examining “families” without exploring variation. If any different forms of family are explored, they typically divide families into couple-parent and sole-parent families.</p> <p>Shining a spotlight on family diversity is essential to ensuring that policies, systems and society are supportive and inclusive of the many ways children and young people grow up. We need to <a href="https://research-information.bris.ac.uk/ws/files/236470646/socsci_09_00083_v2.pdf">change the way</a> we think about family.</p> <p>Our team, in partnership with <a href="https://www.uniting.org/home">Uniting NSW.ACT</a>, will report annually over the next decade to chart the diversity of families, how this changes over time, and the implications for policy and practice. Our first report analyses the 2022 wave of Housing Income and Dynamics in Australia (HILDA) data and the 2021 Census to examine these issues.</p> <p><a href="https://www.uniting.org/content/dam/uniting/documents/families-report/uniting-families-report-2024.pdf">We found</a> while the majority of children live in couple-parent (69%) and sole-parent (11%) families, 12% of children live in step/blended families, 6% live in multigenerational families, and around 1% live in foster families or families made up of other kin.</p> <p>We also found First Nations families are more likely to live in multigenerational (9%), step and/or blended (27%), sole-parent (23%), or foster and other kin (6%) families.</p> <h2>Care, wealth and labour</h2> <p>Our analysis of ten years of the Census shows this diverse mix of families has been a consistent part of the Australian population over time.</p> <p>As well as nurturing children and supporting young adults as they establish themselves in the world, we found families provide care during times of ill-health and disability. Unsurprisingly, multigenerational families are likely to provide this kind of care, because they are living with older adults with care needs.</p> <p>But this was also the case for sole-parent families, step/blended families and foster and other kin families. All these family types are at least twice as likely to provide care than couple families.</p> <p>We know the cost of living is affecting many families. However, our research suggests that couple families, on the whole, have higher incomes (around 1.5 times more) and higher levels of wealth, and are more likely to be able to cope with rising costs than other families.</p> <p>Sole-parent families, step/blended families, multigenerational families and foster and other kin families have lower incomes and are more likely to experience financial hardship, with close to 20% reaching out to friends, family and community for financial help.</p> <p>Across all family types, we found that old patterns around the gendered division of labour are still in force. Women continue to do more housework and more child-rearing than men. Women have this in common across all family types.</p> <p>What’s more, the majority of women (more than two-thirds) report they believe they are doing more than their fair share. Most men, on the other hand, (again, around two-thirds) feel their contributions are about right. Men heading sole-parent families are the main exception to this pattern.</p> <p>These findings challenge conventional notions of family structure and underscore the importance of inclusive support systems policies that recognise and address the multifaceted needs of families. For example, <a href="https://www.servicesaustralia.gov.au/family-tax-benefit">some family assistance programs</a> base access to supports on household incomes, assuming an increase in resources will benefit all families equally, without considering the number of people in the household or the complexity of caring roles they may hold.</p> <h2>Why does this matter?</h2> <p>Greater understanding of family diversity is important in public debate, policy development and service delivery.</p> <p>By thinking about “children and young people and the people who are raising them”, we have developed a new typology of families, which includes: step/blended, multigenerational, and foster and other kin families – groups that are rarely included in quantitative research.</p> <p>The higher care responsibilities of these family types, combined with access to fewer financial resources, shows the importance of ensuring our policies and programs understand more about the many kinds of families who live in Australia. The findings show there is work to do to ensure that conditions and opportunities are equal for all families.</p> <p>At the same time, old challenges about women’s uneven greater share of work raising children, and caring for the homes in which we live, continue to need our focused attention to redress gender imbalances.</p> <p>Importantly, the research also uncovers areas of hope. Despite the obvious challenges many families face, the resilience and care within families is clear. Satisfaction with relationships with children and between siblings is high across all family types.</p> <p>In the next decade we hope to build a compelling narrative that provides a rich evidence base on how family practices, relationships, needs and circumstances change.</p> <p>A better understanding of the rich of tapestry of families and family life in which children and young people are raised in Australia will compel us to look more closely in the design of our policies and systems to disrupt entrenched disadvantage and secure the futures of the next generation.<!-- Below is The Conversation's page counter tag. Prince George looks all grown up in new birthday portrait

<p>Prince George is 11! </p> <p>The Princess of Wales has celebrated her son's birthday by sharing a new portrait of the young royal on social media. </p> <p>“Wishing Prince George a very happy 11th birthday today!” she captioned the black and white photo of George looking all grown up in a button up shirt and black blazer. </p> <p>Royal fans also flooded the comment section with happy birthday wishes for the young monarch. </p> <p>"This picture is so beautiful, he is growing up so fast! I wish the happiest birthday to Prince George, have the best day," one wrote</p> <p>"Prince George looks so much like his Dad," another added.</p> <p>"HE'S GROWING UP SO FAST! I can't believe! Happy birthday George!" a third commented.</p> <p>"Happy birthday! That makes me laugh and it makes George laugh."</p> <p>She also praised the young prince saying:  “He is amazing. David and Victoria Beckham recreate iconic purple wedding look

<p>David and Victoria Beckham have slipped back into the infamous purple wedding outfits to mark their 25th anniversary. </p> <p>The pair have shared a bunch of photos on social media from the lead up to their wedding in July 1999, giving fans a more detailed insight into the big day. </p> <p>"Looking back on the days leading up to our wedding 25 years ago brings back so many amazing memories… I love you so much," Victoria captioned one of the posts, before tagging David. </p> <p>The photos included a picture of the pair testing their iconic thrones for size when they were first delivered, Victoria's final fitting for her wedding dress and David fully concentrating while writing his vows among a few other special moments. </p> <p>In another post, the couple released a series of shots of them sitting on the reupholstered thrones, wearing their iconic Antonio Berardi wedding reception outfits. </p> <p>"Yep, still got it! 🤣 Can’t believe it’s been 25 years and they still fit!" Victoria captioned the photo before tagging all of their kids on social media. </p> <p>It wasn't an easy process getting into clothes from 25 years ago, and the pair even documented the former Manchester United player struggling to get into the suit, as their 12-year-old daughter Harper was heard giggling in the background.</p> <p>In the Instagram story David can be heard saying "I'm in!" before hearing a pop as he put on the waistcoat making everyone burst into laughter.</p> <p>Fans were thrilled to see the post and loved the Beckhams' sense of humour. </p> <p>"This is the only monarchy I recognise. 👑 Happy anniversary, kids," wrote one fan. </p> <p>"Why does this still look as iconic as the first time!!!" added another. </p> <p>"Going to tell my kids these are the Queen and King of England," joked a third. </p> <p><em>Images: Instagram</em></p> <p> </p>

Beware of ‘tax hacks’ to maximise your return this year. The tax office is taking a close look at incorrect claims

<div class="theconversation-article-body"><em><a href="https://theconversation.com/profiles/ann-kayis-kumar-466422">Ann Kayis-Kumar</a>, <a href="https://theconversation.com/institutions/unsw-sydney-1414">UNSW Sydney</a></em></p> <p>For many people a tax refund is a much-anticipated lump sum of money.</p> <p>So, it is understandable Australians will be looking for ways to maximise their returns – particularly we are in a cost-of-living crisis.</p> <p>But, whether you do your own return or use a tax agent, taking risks is not advised.</p> <h2>Be wary of tax hacks</h2> <p>But be wary of “tax hacks” you might hear about from online sources (I’m looking at you, <a href="https://www.afr.com/companies/professional-services/tiktok-gst-fraud-hit-on-tax-office-blows-out-to-4-6b-20230813-p5dw2y">TikTok</a>). Two truisms spring to mind:</p> <p><strong>1. Don’t let the tax tail wag the dog</strong></p> <p>Many tax hacks suggest you spend considerable money on purchases up front to claim tax deductions. But a tax deduction isn’t actually worth the value amount of your spend.</p> <p>For example: let’s say you’re on a taxable income of A$60,000 per year, which puts you roughly in the <a href="https://www.afr.com/politics/how-wealthy-are-you-compared-to-everyone-else-in-eight-charts-20221214-p5c6a8">50th percentile</a> of income earners and means your <a href="https://www.ato.gov.au/tax-rates-and-codes/tax-rates-australian-residents#ato-Australianresidenttaxrates2020to2025">marginal tax rate is 32.5 cents</a>.</p> <p>You might spend $1,000 on a purchase in the hope of getting a sweet $1,000 tax deduction. However, you’re going to be $675 out of pocket. This is because that $1,000 deduction is only worth $325 (because tax is calculated on your taxable income, which is assessable income less allowable deductions).</p> <p>It will be worth even less next year because of the introduction of the <a href="https://www.abc.net.au/news/2024-02-27/stage-three-tax-cut-changes-pass-senate/103519338">revised Stage 3 tax cuts</a> and that’s a good thing because you’ll be paying less tax overall.</p> <p><strong>2. If it’s too good to be true, it probably is</strong></p> <p>Even if you use a registered tax agent (and it’s important to check they are registered by checking <a href="https://www.tpb.gov.au/public-register">the Tax Practitioners’ Board</a>), it’s a common pitfall to think any aggressive deductions they might suggest are their responsibility if the Australian Taxation Office (ATO) comes knocking. That’s not the case.</p> <p>Taxpayers are responsible for errors in returns made by their tax agents, so the ATO will hold you responsible.</p> <p>Indeed, the <a href="https://www.ato.gov.au/media-centre/ato-flags-3-key-focus-areas-for-this-tax-time">ATO has announced</a> it will be taking a close look at three common errors being made by taxpayers:</p> <ul> <li> <p>incorrectly claiming work-related expenses</p> </li> <li> <p>inflating claims for rental properties</p> </li> <li> <p>failing to include all income when lodging.</p> </li> </ul> <p>It might be tempting to think you’ve got away with over claiming deductions or under reporting income but the ATO has sophisticated systems to <a href="https://www.ato.gov.au/About-ATO/Commitments-and-reporting/Information-and-privacy/How-we-use-data-and-analytics">analyse your data</a>) and track your claims.</p> <p>You’ll need to substantiate your claims, so keep records. If the tax office finds mistakes, you could face <a href="https://www.ato.gov.au/individuals-and-families/paying-the-ato/interest-and-penalties/penalties/penalties-for-making-false-or-misleading-statements">financial penalties</a>, even jail time.</p> <p>Two months ago, a woman was sentenced to two years and six months jail and ordered to repay $39,600 after she lodged three fraudulent Business Activity Statements and received a GST refund to which she wasn’t entitled. While under investigation, she then sent eight false statements to the ATO and tried to claim more money.</p> <p>This is one on many individuals named on the <a href="https://www.ato.gov.au/about-ato/tax-avoidance/the-fight-against-tax-crime/our-focus/refund-fraud/gst-refund-fraud-attempts/operation-protego">ATO’s website</a> highlighting the results of regular crackdowns.</p> <h2>So, should I use a tax agent?</h2> <p>There are nearly 20.5 million active tax file numbers registered to individuals in Australia and last tax year the ATO received 13.7 million individual tax return lodgements. This was a 3% increase on the previous year. Of these lodgements more than 5.6 million were lodged by self-preparers and more than 8 million were lodged by tax agents.</p> <p>It <a href="https://theconversation.com/does-paying-for-tax-advice-save-money-only-if-youre-wealthy-184641">makes sense</a> most Australians use agents to prepare and lodge their tax returns. It’s easier, less stressful, gives you confidence the job is being done right and saves time.</p> <p>Having said that, it does come at a price (see above on the value of deductions), and previous research which finds that <a href="https://theconversation.com/does-paying-for-tax-advice-save-money-only-if-youre-wealthy-184641">every extra dollar spent on a tax agent</a> only yields an estimated tax savings of 20 cents), and if you have simple tax affairs then it’s relatively easy and quick to do it yourself.</p> <h2>How do I prepare my tax return?</h2> <p>Generally, everyone should be lodging an income tax return each year (or, if you don’t need to lodge a tax return, lodging a non-lodgement advice). The ATO has a “Do I need to lodge a tax return?” tool <a href="https://www.ato.gov.au/individuals-and-families/your-tax-return/before-you-prepare-your-tax-return/work-out-if-you-need-to-lodge-a-tax-return">if you’re unsure</a>.</p> <p>It also has a useful <a href="https://www.ato.gov.au/individuals-and-families/your-tax-return/how-to-lodge-your-tax-return/lodge-your-tax-return-online-with-mytax">two minute video</a> which steps you through the process for lodging with their online system myTax.</p> <p>For those of us with simple tax affairs, you just need to follow these steps:</p> <ol> <li> <p>gather and prepare all your information regarding income from work, interest, dividends and any other income such as capital gains from crypto assets or sale of shares</p> </li> <li> <p>then gather and prepare all your information on deductions and work expenses to be claimed making sure you have the evidence to back up your claims. This can be in the form receipts, invoices, log books and diary entries</p> </li> <li> <p>if you are a self-preparer you can log onto your myGov or the ATO’s app to prepare and lodge your return. If you wait until late-July you’ll have the benefit of the ATO’s pre-filled data, too. This gives you plenty of time to make the October 31 deadline.</p> </li> </ol> <p>There’s also the option to use the ATO’s free, volunteer-run TaxHelp program (provided you meet the <a href="https://www.ato.gov.au/individuals-and-families/your-tax-return/help-and-support-to-lodge-your-tax-return/tax-help-program">eligibility criteria</a>), your local Tax Clinic (<a href="https://www.ato.gov.au/individuals-and-families/financial-difficulties-and-disasters/support-to-lodge-and-pay/national-tax-clinic-program">details here</a>), or by seeking help from a registered tax agent. Just make sure you engage them before the October 31 deadline.</p> <h2>Where it might get tricky</h2> <p>But for others, for example if you have an ABN, it gets a bit more complicated. If you operate your business as a sole trader, you must lodge a tax return, even if your income is below the tax-free threshold.</p> <p>And if you have registered for GST – which you must do when your business or enterprise has a GST turnover of $75,000 or more, or if you are a taxi driver or Uber driver – then you will also need to submit quarterly BAS.</p> <p>It gets even more complicated for partnerships, trusts and companies, so it is best to seek the guidance and professional expertise of a registered tax agent, if you aren’t already.</p> <h2>What if I can’t afford a tax agent?</h2> <p>This year, many Australians are doing it tough. Indeed, research by the ASIC’s Moneysmart program estimates <a href="https://www.abc.net.au/news/2024-06-04/asic-survey-millions-of-australians-facing-financial-difficulty/103926704">more than five million Australians</a> are in financial strife.</p> <p>Many people will find it hard to prioritise paying a registered tax agent when they cannot afford basic necessities like food.</p> <p>If you’re in this situation, you might find it useful to get in touch with a free financial counsellor via the <a href="https://ndh.org.au/">National Debt Helpline</a> or the <a href="https://sbdh.org.au/">Small Business Debt Helpline</a>.</p> <h2>Don’t procrastinate</h2> <p>Don’t put off doing your tax. If you’re behind, it might seem daunting to get back on track, especially if you think you’ll have to pay extra tax this year instead of getting a refund. But not lodging your returns will backfire. Like avoiding a trip to the doctor to get a skin check, the longer you wait, the more the problem will grow.</p> <p>Reaching out to the ATO is the key because they have tools to support you, including payment plans. It also shows the ATO that you are willing to comply. "Looks nothing like her": Kate Middleton portrait ridiculed

Do they have a recurrent gesture?"</p> <p>“[Kate] has really risen up to her role – she was born for this. All the head-turning looks from the 2024 Met Gala

<p>Known as "fashion's biggest night out", the Met Gala 2024 has kicked off in spectacular style with A-listers from all over the world gracing the carpet at the Metropolitan Museum of Art in New York City. </p> <p>The event, which is a fundraising event for the Met, is held every year on the first Monday of May, to celebrate the Costume Institute’s new exhibition, “Sleeping Beauties: Reawakening Fashion”.</p> <p>The dress code this year, The Garden of Time, is said to be inspired by a short story of the same title written by JG Ballard in 1962. </p> <p>The who's who of Hollywood hit the carpet at the Met this year, led by actress Zendaya, who is this year's co-chair of the event after returning to the Gala for the first time in five years. </p> <p>Many Aussie superstars walked the carpet, such as Nicole Kidman and Keith Urban, Naomi Watts, Troye Sivan, Chris Hemsworth and Kylie Minogue, who attended for the first time since 2014.</p> <p>Hugh Jackman also graced the Met carpet solo for the first time, last attending alongside his now ex-wife Deborra Lee-Furness in 2023. </p> <p>The Aussie actor took to Instagram to share that his dapper Tom Ford tuxedo was the very same outfit that he wore to his first Met Gala in 2004 that had been "refitted and repaired". </p> <p>Other Hollywood legends that graced the carpet included Sarah Jessica Parker, Meg Ryan, Jennifer Lopez, Uma Thurman, Penelope Cruz and many more. </p> <p><em>Image credits: Getty Images </em></p>

To tackle gendered violence, we also need to look at drugs, trauma and mental health

<div class="theconversation-article-body"> <p><em><a href="https://theconversation.com/profiles/siobhan-odean-1356613">Siobhan O'Dean</a>, <a href="https://theconversation.com/institutions/university-of-sydney-841">University of Sydney</a>; <a href="https://theconversation.com/profiles/lucinda-grummitt-1531503">Lucinda Grummitt</a>, <a href="https://theconversation.com/institutions/university-of-sydney-841">University of Sydney</a>, and <a href="https://theconversation.com/profiles/steph-kershaw-1466426">Steph Kershaw</a>, <a href="https://theconversation.com/institutions/university-of-sydney-841">University of Sydney</a></em></p> <p>After several highly publicised alleged murders of women in Australia, the Albanese government this week pledged <a href="https://ministers.pmc.gov.au/gallagher/2024/helping-women-leave-violent-partner-payment">more than A$925 million</a> over five years to address men’s violence towards women. This includes up to $5,000 to support those escaping violent relationships.</p> <p>However, to reduce and prevent gender-based and intimate partner violence we also need to address the root causes and contributors. These include alcohol and other drugs, trauma and mental health issues.</p> <h2>Why is this crucial?</h2> <p>The World Health Organization estimates <a href="https://iris.who.int/bitstream/handle/10665/341604/WHO-SRH-21.6-eng.pdf?sequence=1">30% of women</a> globally have experienced intimate partner violence, gender-based violence or both. In Australia, <a href="https://www.abs.gov.au/statistics/people/crime-and-justice/partner-violence/latest-release#key-statistics">27% of women</a> have experienced intimate partner violence by a co-habiting partner; <a href="https://pubmed.ncbi.nlm.nih.gov/37004184/">almost 40%</a> of Australian children are exposed to domestic violence.</p> <p>By gender-based violence we mean violence or intentionally harmful behaviour directed at someone due to their gender. But intimate partner violence specifically refers to violence and abuse occurring between current (or former) romantic partners. Domestic violence can extend beyond intimate partners, to include other family members.</p> <p>These statistics highlight the urgent need to address not just the aftermath of such violence, but also its roots, including the experiences and behaviours of perpetrators.</p> <h2>What’s the link with mental health, trauma and drugs?</h2> <p>The relationships between mental illness, drug use, traumatic experiences and violence are complex.</p> <p>When we look specifically at the link between mental illness and violence, most people with mental illness will not become violent. But there <a href="https://theconversation.com/bondi-attacker-had-mental-health-issues-but-most-people-with-mental-illness-arent-violent-227868">is evidence</a> people with serious mental illness can be more likely to become violent.</p> <p>The use of alcohol and other drugs also <a href="https://theconversation.com/alcohol-and-drug-use-exacerbate-family-violence-and-can-be-dealt-with-69986">increases the risk</a> of domestic violence, including intimate partner violence.</p> <p>About <a href="https://www.aihw.gov.au/family-domestic-and-sexual-violence/understanding-fdsv/factors-associated-with-fdsv">one in three</a> intimate partner violence incidents involve alcohol. These are more likely to result in physical injury and hospitalisation. The risk of perpetrating violence is even higher for people with mental ill health who are also <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC1525086/">using alcohol or other drugs</a>.</p> <p>It’s also important to consider traumatic experiences. Most people who experience trauma do not commit violent acts, but there are <a href="https://www.thelancet.com/journals/lanpub/article/PIIS2468-2667(23)00075-0/fulltext">high rates</a> of trauma among people who become violent.</p> <p>For example, experiences of childhood trauma (such as witnessing physical abuse) <a href="https://www.sciencedirect.com/science/article/pii/S1359178915000828?via%3Dihub">can increase the risk</a> of perpetrating domestic violence as an adult.</p> <p>Early traumatic experiences can affect the brain and body’s <a href="https://bmcmedicine.biomedcentral.com/articles/10.1186/s12916-017-0895-4">stress response</a>, leading to heightened fear and perception of threat, and difficulty regulating emotions. This can result in aggressive responses when faced with conflict or stress.</p> <p>This response to stress increases the risk of <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9675346/">alcohol and drug problems</a>, developing <a href="https://pubmed.ncbi.nlm.nih.gov/30798897/">PTSD</a> (post-traumatic stress disorder), and <a href="https://psycnet.apa.org/record/2015-17349-001">increases the risk</a> of perpetrating intimate partner violence.</p> <h2>How can we address these overlapping issues?</h2> <p>We can reduce intimate partner violence by addressing these overlapping issues and tackling the root causes and contributors.</p> <p>The early intervention and treatment of <a href="https://doi.org/10.1186/s12905-019-0728-z">mental illness</a>, <a href="https://doi.org/10.1177/1541204020939645">trauma</a> (including PTSD), and <a href="https://doi.org/10.1016/j.avb.2015.06.001">alcohol and other drug use</a>, could help reduce violence. So extra investment for these are needed. We also need more investment to <a href="https://www.sciencedirect.com/science/article/pii/S2212657023000508">prevent mental health issues</a>, and preventing alcohol and drug use disorders from developing in the first place.</p> <p><a href="https://www.sciencedirect.com/science/article/pii/S074937972200023X?via%3Dihub">Preventing trauma</a> from occuring and supporting those exposed is crucial to end what can often become a vicious cycle of intergenerational trauma and violence. <a href="https://journals.sagepub.com/doi/10.1177/070674371105600505">Safe and supportive</a> environments and relationships can protect children against mental health problems or further violence as they grow up and engage in their own intimate relationships.</p> <p>We also need to acknowledge the widespread <a href="https://store.samhsa.gov/product/practical-guide-implementing-trauma-informed-approach/pep23-06-05-005">impact of trauma</a> and its effects on mental health, drug use and violence. This needs to be integrated into policies and practices to reduce re-traumatising individuals.</p> <h2>How about programs for perpetrators?</h2> <p>Most existing standard intervention programs for perpetrators <a href="https://journals.sagepub.com/doi/10.1177/1524838018791268">do not consider</a> the links between trauma, mental health and perpetrating intimate partner violence. Such programs tend to have <a href="https://psycnet.apa.org/doi/10.1037/a0012718">little</a> or <a href="https://doi.org/10.1016/j.cpr.2021.101974">mixed effects</a> on the behaviour of perpetrators.</p> <p>But we could improve these programs with a <a href="http://rcfv.archive.royalcommission.vic.gov.au/MediaLibraries/RCFamilyViolence/Reports/RCFV_Full_Report_Interactive.pdf">coordinated approach</a> including treating mental illness, drug use and trauma at the same time.</p> <p>Such “<a href="https://www.sciencedirect.com/science/article/pii/S014976341930449X?via%3Dihub">multicomponent</a>” programs show promise in meaningfully reducing violent behaviour. However, we need more rigorous and large-scale evaluations of how well they work.</p> <h2>What needs to happen next?</h2> <p>Supporting victim-survivors and improving interventions for perpetrators are both needed. However, intervening once violence has occurred is arguably too late.</p> <p>We need to direct our efforts towards broader, holistic approaches to prevent and reduce intimate partner violence, including addressing the underlying contributors to violence we’ve outlined.</p> <p>We also need to look more widely at preventing intimate partner violence and gendered violence.</p> <p>We need developmentally appropriate <a href="https://theconversation.com/4-things-our-schools-should-do-now-to-help-prevent-gender-based-violence-228993">education and skills-based programs</a> for adolescents to prevent the emergence of unhealthy relationship patterns before they become established.</p> <p>We also need to address the <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7278040/">social determinants of health</a> that contribute to violence. As you travel, pause and take a look at airport chapels

<div class="theconversation-article-body"> <p><em><a href="https://theconversation.com/profiles/wendy-cadge-343734">Wendy Cadge</a>, <a href="https://theconversation.com/institutions/brandeis-university-1308">Brandeis University</a></em></p> <p>Flying home? It is very likely there is a chapel or meditation room tucked away somewhere in one of the airports you’ll pass through. <a href="http://www.pewresearch.org/fact-tank/2015/07/06/most-of-the-busiest-u-s-airports-have-dedicated-chapels/">Sixteen of the country’s 20 largest airports</a> have chapels, as do many more around the world.</p> <p>I am a <a href="http://www.wendycadge.com/">sociologist</a> of contemporary American religion and have written <a href="http://www.wendycadge.com/publications/airport-chapels-and-chaplains/">two recent articles</a> about airport chaplains and chapels. My interest in airport chapels started as simple curiosity – why do airports have chapels and who uses them? After visiting a few – including the chapel at Logan, my home airport here in Boston – I have concluded that they reflect broader changing norms around American religion.</p> <h2>How airports came to have chapels</h2> <p>The country’s first airport chapels were intended for staff rather than passengers and were established by Catholic leaders in the 1950s and 1960s to make sure their parishioners could attend mass.</p> <p>The first one in the U.S., Our Lady of the Airways, was built by Boston Archbishop Richard J. Cushing at Logan airport in 1951 and it was explicitly meant for people working at the <a href="https://doi.org/10.1093/socrel/srx025">airport</a>. A neon light pointed to the chapel and souvenir cards handed out at the dedication read, “We fly to thy patronage, O Holy Mother of God; despise not our petitions in our necessities, but deliver us away from all dangers, O glorious and blessed virgin.”</p> <p>Our Lady of the Airways inspired the building of the country’s second airport chapel, Our Lady of the Skies at what was then Idlewild – and is today John F. Kennedy airport in New York City.</p> <p>Protestant chapels came later. The first was in New York – again at JFK. It was designed in the shape of a Latin cross and was joined by a Jewish synagogue in the 1960s. These chapels were located at a distance from the terminals: Passengers wishing to visit them had to go outside. They were <a href="https://books.google.com/books/about/Exploring_Interfaith_Space.html?id=on5YNwAACAA">later razed</a> and rebuilt in different area of JFK.</p> <p>In the 1970s and 1980s, Protestant chapels opened in Atlanta, and in several terminals of the Dallas airport in Texas.</p> <h2>Becoming more inclusive</h2> <p>By the 1990s and 2000s, single faith chapels had become a <a href="http://www.tciarchive.org/4534.article">“dying breed.”</a> Most started to welcome people from all religions. And many were transformed into spaces for reflection, or meditation for weary travelers.</p> <p>The chapel at San Francisco International Airport, for example, known as the <a href="https://www.flysfo.com/content/berman-reflection-room-0">Berman Reflection Room</a> for Jewish philanthropist Henry Berman who was a former president of the San Francisco Airport Commission, looks like a quiet waiting room filled with plants and lines of connected chairs. A small enclosed space without any religious symbols or obvious connections to things religious or spiritual is available for services.</p> <p>The scene at the <a href="http://www.atlchapel.org/">Atlanta</a> airport chapel is similar, with only a few chairs and clear glass entrances, to provide space for quiet reflection.</p> <p>Some airports, such as JFK, continue with their “Our Lady” names, indicating their faith-based origins.</p> <p>Others include religious symbols and objects from a range of religious traditions. The chapel in <a href="https://cltairportchapel.org/">Charlotte</a>, North Carolina, for example, has multiple religious texts alongside prayer rugs, rosary beads and artistically rendered quotes from the world’s major religions.</p> <p>Pamphlets on topics ranging from grief to forgiveness are available for visitors to take with them at the Charlotte airport.</p> <h2>Different airports, different rules</h2> <p>As these examples show, <a href="https://doi.org/10.1093/socrel/srx025">no two airports</a> have negotiated chapel space in the same way. What is permissible in one city is often not in another. Often, it is local, historical and demographic factors, including the religious composition of the region, that influence decisions. These could even be based on who started the chapel, or how much interreligious cooperation there is in a city.</p> <p>Certain airports such as Chicago’s <a href="http://www.airportchapels.org/">O'Hare</a> have strict rules regarding impromptu religious gatherings whether inside the chapel or out. Some use their public address systems to announce religious services. Others prohibit such announcements and do not even allow airport chaplains to put out any signs that could indicate a religious space.</p> <p>If they are included in airport maps, chapels tend to be designated by the symbol of a person bent in prayer. But even then, they can be difficult to spot. About half of the existing chapels are on the pre-security side of the airport and the other half accessible only after passengers pass through security.</p> <p><a href="https://doi.org/10.1093/socrel/srx025">Only four large American airports</a> – Las Vegas, Los Angeles, Philadelphia and New York’s LaGuardia – do not have chapel spaces, although opening such a space is under consideration. In the interim, at LaGuardia, a Catholic chaplain holds mass in a conference room.</p> <h2>What’s the future?</h2> <p>The reasons for these spaces and their variations are idiosyncratic and intensely local. These chapels reveal a range of approaches to contemporary American religion and spirituality.</p> <p>So on your travels, keep an eye out for these chapels. Note their similarities and differences and recognize how important local histories are to how church-state issues are resolved – at airports and beyond.<!-- Below is The Conversation's page counter tag. How to look after your mental health while packing up Mum or Dad’s home

It’s a huge job and you’re dreading it.</p> <p>It’s normal to feel grief, loss, guilt, exhaustion or even resentment at being left with this job.</p> <p>So how can you look after your mental health while tackling the task?</p> <h2>It’s OK to feel a lot of feelings</h2> <p>Research has documented how this task can exert an intense <a href="https://www.tandfonline.com/doi/full/10.1080/15267431.2021.1943399">physical and emotional toll</a>.</p> <p>This can be more intense for those who had strained – or even <a href="https://journals.sagepub.com/doi/epub/10.1177/0030222819868107">traumatic</a> – relationships with the person whose house they’re packing up.</p> <p>Decisions around distributing or discarding items can, in some families, bring up painful reminders of the past or end up <a href="https://journals.sagepub.com/doi/10.1177/1074840711428451">replaying strained dynamics</a>.</p> <p>Family members who were carers for the deceased may feel exhaustion, overwhelm, <a href="https://onlinelibrary.wiley.com/doi/abs/10.1002/hec.1512?sid=vendor%3Adatabase">burnout</a> or a sense of injustice they must now continue to be responsible for their loved one’s affairs. Grief can be compounded by the practical challenges of deciding how to <a href="https://doi.org/10.1016/0148-2963(94)00054-I">store or discard belongings</a>, <a href="https://www.jstor.org/stable/30000385">arrange the funeral</a>, execute the will, deal with the aged care place or, in some cases, navigate legal disputes.</p> <p>But packing up the house may also be cathartic or helpful. <a href="https://www.tandfonline.com/doi/full/10.1080/15267431.2021.1943399">Research</a> has shown how the task of cleaning out a loved one’s belongings can provide an opportunity for family and friends to talk, share memories, and make sense of what has just happened.</p> <p>It’s also normal to grieve before someone dies. What psychologists call “<a href="https://pubmed.ncbi.nlm.nih.gov/29206700/">anticipatory</a> grief” can happen to <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC1615888/">relatives packing up the house</a> of a parent who has moved to aged care or palliative care.</p> <h2>What to do with all this stuff?</h2> <p><a href="https://doi.org/10.1016/0148-2963(94)00054-I">Some</a> treat their loved one’s items with sanctity, holding onto as many of their belongings as possible and creating “shrines” in their honour.</p> <p>Others alleviate the weight of grief by clearing out a loved one’s house as soon as possible, giving away, selling or discarding as much as they can.</p> <p>But if you experience a mix of these – enthusiastically getting rid of some stuff, while desperately wanting to hold onto other things – that’s OK too.</p> <p>One <a href="https://www.tandfonline.com/doi/abs/10.1080/10253866.2017.1367677">study</a> identified a process punctuated by four key periods:</p> <ol> <li> <p>numbness and overwhelm at the task of packing the house</p> </li> <li> <p>yearning to maintain a link to the loved through their belongings</p> </li> <li> <p>working through grief, anger and guilt regarding the loved one and the task of managing their belongings, and</p> </li> <li> <p>healing and making sense of the relationship with the deceased and their belongings.</p> </li> </ol> <p>However, it is important to note everyone’s approach is different and there is no “right” way to do the clean out, or “right” way to feel.</p> <h2>Caring for your mental health during the clean out</h2> <p>To care for your mental health during these difficult times, you might try to:</p> <ul> <li> <p>make space for your feelings, whether it’s sadness, loss, resentment, anger, relief or all the above. There is no right or wrong way to feel. <a href="https://guilfordjournals.com/doi/abs/10.1521/jscp.2011.30.2.163">Accepting</a> your emotions is healthier than suppressing them</p> </li> <li> <p>share the load. <a href="https://onlinelibrary.wiley.com/doi/abs/10.1046/j.1365-2648.1999.01220.x">Research</a> has shown practical support from close friends and family can help a lot with grief. Accept help with packing, planning, dealing with removalists, selling or donating items and cleaning. Don’t be afraid to reduce your mental load by delegating tasks to friends, who are likely wondering how they can help</p> </li> <li> <p>take a systematic approach. Break tasks into their smallest component. For example, aim to clean out a drawer instead of an entire bedroom. This can help the mental and physical task feel more manageable</p> </li> <li> <p>reflect on what’s meaningful to you. Some belongings will have <a href="https://doi.org/10.1016/0148-2963(94)00054-I">meaning</a>, while others will not. What was valuable to the deceased may not be valuable to you. Things they probably saw as pretty worthless (a handwritten shopping list, an old sewing kit) may be very meaningful to you. Ask yourself whether retaining a small number of meaningful possessions would allow you to maintain a connection with your loved one, or if clearing out the space and discarding the items is what you need</p> </li> <li> <p>share your story. When you feel ready, share your “<a href="https://www.tandfonline.com/doi/full/10.1080/15267431.2021.1943399">cleaning out the closet</a>” story with trusted friends and family. Storytelling allows the deceased to live on in memory. <a href="https://onlinelibrary.wiley.com/doi/abs/10.1046/j.1365-2648.1999.01220.x">Research</a> also suggests we cope better with bereavement when friends and relatives make time to hear our feelings</p> </li> <li> <p>remember that professional help is available. Just as a solicitor can help with legal disputes, a mental health professional can help you process your feelings.</p> </li> </ul> <p>The home of your loved one is not merely a place where they lived, but a space filled with meaning and stories.</p> <p>Packing up the house of a loved one can be incredibly daunting and challenging, but it can also be an important part of your grieving process.</p> <p><em>If this article has raised issues for you, or if you’re concerned about someone you know, call Lifeline on 13 11 14.</em><!-- Below is The Conversation's page counter tag. Who will look after us in our final years? A pay rise alone won’t solve aged-care workforce shortages

The federal government <a href="https://ministers.dewr.gov.au/burke/fair-work-decision-aged-care">has committed to</a> the increases, but is yet to announce when they will start.</p> <blockquote class="twitter-tweet"> <p dir="ltr" lang="en">Tens of thousands of aged care workers will receive a major pay rise after the Fair Work Commission recommended the increase. <a href="https://t.co/NeNt1Gvxd9">https://t.co/NeNt1Gvxd9</a></p> <p>— SBS News (@SBSNews) <a href="https://twitter.com/SBSNews/status/1768557710537068889?ref_src=twsrc%5Etfw">March 15, 2024</a></p></blockquote> <p>But while wage rises for aged-care workers are welcome, this measure alone will not fix all workforce problems in the sector. The number of people over 80 is expected to <a href="https://treasury.gov.au/sites/default/files/2023-08/p2023-435150.pdf">triple over the next 40 years</a>, driving an increase in the number of aged care workers needed.</p> <h2>How did we get here?</h2> <p>The Royal Commission into Aged Care Quality and Safety, which delivered its <a href="https://www.royalcommission.gov.au/aged-care/final-report">final report</a> in March 2021, identified a litany of tragic failures in the regulation and delivery of aged care.</p> <p>The former Liberal government was dragged reluctantly to accept that a total revamp of the aged-care system was needed. But its <a href="https://www.health.gov.au/ministers/the-hon-greg-hunt-mp/media/respect-care-and-dignity-aged-care-royal-commission-452-million-immediate-response-as-government-commits-to-historic-reform-to-deliver-respect-and-care-for-senior-australians#:%7E:text=Minister%20for%20Senior%20Australians%20and,%2C%20dementia%2C%20food%20and%20nutrition.">weak response</a> left the heavy lifting to the incoming Labor government.</p> <p>The current government’s response started well, with a <a href="https://theconversation.com/anthony-albanese-offers-2-5-billion-plan-to-fix-crisis-in-aged-care-180419">significant injection of funding</a> and a promising regulatory response. But it too has failed to pursue a visionary response to the problems identified by the Royal Commission.</p> <p>Action was needed on four fronts:</p> <ul> <li>ensuring enough staff to provide care</li> <li>building a functioning regulatory system to encourage good care and weed out bad providers</li> <li>designing and introducing a fair payment system to distribute funds to providers and</li> <li>implementing a financing system to pay for it all and achieve intergenerational equity.</li> </ul> <p>A government taskforce which proposed a <a href="https://theconversation.com/what-will-aged-care-look-like-for-the-next-generation-more-of-the-same-but-higher-out-of-pocket-costs-225551">timid response to the fourth challenge</a> – an equitable financing system – was released at the start of last week.</p> <p>Consultation closed on a <a href="https://media.opan.org.au/uploads/2024/03/240308_Aged-Care-Act-Exposure-Draft-Joint-Submission_FINAL.pdf">very poorly designed new regulatory regime</a> the week before.</p> <p>But the big news came at end of the week when the Fair Work Commission handed down a further <a href="https://www.fwc.gov.au/documents/decisionssigned/pdf/2024fwcfb150.pdf">determination</a> on what aged-care workers should be paid, confirming and going beyond a previous <a href="https://www.fwc.gov.au/documents/sites/work-value-aged-care/decisions-statements/2022fwcfb200.pdf">interim determination</a>.</p> <h2>What did the Fair Work Commission find?</h2> <p>Essentially, the commission determined that work in industries with a high proportion of women workers has been traditionally undervalued in wage-setting. This had consequences for both care workers in the aged-care industry (nurses and <a href="https://training.gov.au/Training/Details/CHC33021">Certificate III-qualified</a> personal-care workers) and indirect care workers (cleaners, food services assistants).</p> <p>Aged-care staff will now get significant pay increases – 18–28% increase for personal care workers employed under the Aged Care Award, inclusive of the increase awarded in the interim decision.</p> <figure class="align-center "><figcaption></figcaption>Indirect care workers were awarded a general increase of 3%. Laundry hands, cleaners and food services assistants will receive a further 3.96% <a href="https://www.fwc.gov.au/documents/decision-summaries/2024fwcfb150-summary.pdf">on the grounds</a> they “interact with residents significantly more regularly than other indirect care employees”.</figure> <p>The final increases for registered and enrolled nurses will be determined in the next few months.</p> <h2>How has the sector responded?</h2> <p>There has been no push-back from employer groups or conservative politicians. This suggests the uplift is accepted as fair by all concerned.</p> <p>The interim increases of up to 15% probably facilitated this acceptance, with the <a href="https://theconversation.com/what-does-the-budget-mean-for-medicare-medicines-aged-care-and-first-nations-health-192842">recognition of the community</a> that care workers should be paid more than fast food workers.</p> <p>There was <a href="https://www.accpa.asn.au/media-releases/accpa-welcomes-further-aged-care-wage-rises">no criticism from aged-care providers</a> either. This is probably because they are facing difficulty in recruiting staff at current wage rates. And because government payments to providers reflect the <a href="https://www.ihacpa.gov.au/">actual cost of aged care</a>, increased payments will automatically flow to providers.</p> <p>When the increases will flow has yet to be determined. The government is due to give its recommendations for staging implementation by mid-April.</p> <h2>Is the workforce problem fixed?</h2> <p>An increase in wages is necessary, but alone is not sufficient to solve workforce shortages.</p> <p>The health- and social-care workforce is <a href="https://www.jobsandskills.gov.au/data/employment-projections">predicted</a> to grow faster than any other sector over the next decade. The “care economy” will <a href="https://theconversation.com/care-economy-to-balloon-in-an-australia-of-40-5-million-intergenerational-report-211876">grow</a> from around 8% to around 15% of GDP over the next 40 years.</p> <p>This means a greater proportion of school-leavers will need to be attracted to the aged-care sector. Aged care will also need to attract and retrain workers displaced from industries in decline and attract suitably skilled migrants and refugees with appropriate language skills.</p> <p>The <a href="https://theconversation.com/demand-driven-funding-for-universities-is-frozen-what-does-this-mean-and-should-the-policy-be-restored-116060">caps on university and college enrolments</a> imposed by the previous government, coupled with weak student demand for places in key professions (such as nursing), has meant workforce shortages will continue for a few more years, despite the allure of increased wages.</p> <p>A significant increase in intakes into university and vocational education college courses preparing students for health and social care is still required. Better pay will help to increase student demand, but funding to expand place numbers will ensure there are enough qualified staff for the aged-care system of the future. <!-- Below is The Conversation's page counter tag. What will aged care look like for the next generation?

<p><a href="https://theconversation.com/profiles/hal-swerissen-9722">Hal Swerissen</a>, <em><a href="https://theconversation.com/institutions/la-trobe-university-842">La Trobe University</a></em></p> <p>Aged care financing is a vexed problem for the Australian government. It is already underfunded for the quality the community expects, and costs will increase dramatically. There are also significant concerns about the complexity of the system.</p> <p>In 2021–22 the federal government spent <a href="https://www.health.gov.au/resources/publications/final-report-of-the-aged-care-taskforce?language=en">A$25 billion</a> on aged services for around 1.2 million people aged 65 and over. Around 60% went to residential care (<a href="https://www.gen-agedcaredata.gov.au/topics/people-using-aged-care#:%7E:text=On%2030%20June%202022%2C%20approximately,and%203%2C500%20using%20transition%20care.">190,000 people</a>) and one-third to home care (<a href="https://www.gen-agedcaredata.gov.au/topics/people-using-aged-care#:%7E:text=On%2030%20June%202022%2C%20approximately,and%203%2C500%20using%20transition%20care.">one million people</a>).</p> <p>The <a href="https://www.health.gov.au/resources/publications/final-report-of-the-aged-care-taskforce?language=en">final report from the government’s Aged Care Taskforce</a>, which has been reviewing funding options, estimates the number of people who will need services is likely to grow to more than two million over the next 20 years. Costs are therefore likely to more than double.</p> <p>The taskforce has considered what aged care services are reasonable and necessary and made recommendations to the government about how they can be paid for. This includes getting aged care users to pay for more of their care.</p> <p>But rather than recommending an alternative financing arrangement that will safeguard Australians’ aged care services into the future, the taskforce largely recommends tidying up existing arrangements and keeping the status quo.</p> <h2>No Medicare-style levy</h2> <p>The taskforce <a href="https://www.health.gov.au/resources/publications/final-report-of-the-aged-care-taskforce?language=en">rejected</a> the aged care royal commission’s recommendation to introduce a levy to meet aged care cost increases. A 1% levy, similar to the Medicare levy, could have raised around <a href="https://www.thenewdaily.com.au/finance/finance-news/2021/03/03/cost-of-aged-care-levy#:%7E:text=Overall%2C%20a%201%20per%20cent%20levy%20would%20raise,necessary%20to%20provide%20decent%20aged%20care%20for%20all.">$8 billion a year</a>.</p> <p>The taskforce failed to consider the mix of taxation, personal contributions and social insurance which are commonly used to fund aged care systems internationally. The <a href="https://www.oecd.org/els/health-systems/Japan-OECD-EC-Good-Time-in-Old-Age.pdf">Japanese system</a>, for example, is financed by long-term insurance paid by those aged 40 and over, plus general taxation and a small copayment.</p> <p>Instead, the taskforce puts forward a simple, pragmatic argument that older people are becoming wealthier through superannuation, there is a cost of living crisis for younger people and therefore older people should be required to pay more of their aged care costs.</p> <h2>Separating care from other services</h2> <p>In deciding what older people should pay more for, the taskforce divided services into care, everyday living and accommodation.</p> <p>The taskforce thought the most important services were clinical services (including nursing and allied health) and these should be the main responsibility of government funding. Personal care, including showering and dressing were seen as a middle tier that is likely to attract some co-payment, despite these services often being necessary to maintain independence.</p> <p>The task force recommended the costs for everyday living (such as food and utilities) and accommodation expenses (such as rent) should increasingly be a personal responsibility.</p> <h2>Making the system fairer</h2> <p>The taskforce thought it was unfair people in residential care were making substantial contributions for their everyday living expenses (about 25%) and those receiving home care weren’t (about 5%). This is, in part, because home care has always had a muddled set of rules about user co-payments.</p> <p>But the taskforce provided no analysis of accommodation costs (such as utilities and maintenance) people meet at home compared with residential care.</p> <p>To address the inefficiencies of upfront daily fees for packages, the taskforce recommends means testing co-payments for home care packages and basing them on the actual level of service users receive for everyday support (for food, cleaning, and so on) and to a lesser extent for support to maintain independence.</p> <p>It is unclear whether clinical and personal care costs and user contributions will be treated the same for residential and home care.</p> <h2>Making residential aged care sustainable</h2> <p>The taskforce was <a href="https://www.health.gov.au/resources/publications/final-report-of-the-aged-care-taskforce?language=en">concerned</a> residential care operators were losing $4 per resident day on “hotel” (accommodation services) and everyday living costs.</p> <p>The taskforce recommends means tested user contributions for room services and everyday living costs be increased.</p> <p>It also recommends that wealthier older people be given more choice by allowing them to pay more (per resident day) for better amenities. This would allow providers to fully meet the cost of these services.</p> <p>Effectively, this means daily living charges for residents are too low and inflexible and that fees would go up, although the taskforce was clear that low-income residents should be protected.</p> <h2>Moving from buying to renting rooms</h2> <p>Currently older people who need residential care have a choice of making a refundable up-front payment for their room or to pay rent to offset the loans providers take out to build facilities. Providers raise capital to build aged care facilities through equity or loan financing.</p> <p>However, the taskforce did not consider the overall efficiency of the private capital market for financing aged care or alternative solutions.</p> <p>Instead, it recommended capital contributions be streamlined and simplified by phasing out up-front payments and focusing on rental contributions. This echoes the royal commission, which found rent to be a more efficient and less risky method of financing capital for aged care in private capital markets.</p> <p>It’s likely that in a decade or so, once the new home care arrangements are in place, there will be proportionally fewer older people in residential aged care. Those who do go are likely to be more disabled and have greater care needs. And those with more money will pay more for their accommodation and everyday living arrangements. But they may have more choice too.</p> <p>Although the federal government has <a href="https://www.abc.net.au/news/2024-03-11/aged-care-task-force-hands-down-recommendations/103573554">ruled out an aged care levy</a> and <a href="https://www.abc.net.au/news/2024-02-15/no-plan-to-touch-aged-care-asset-test/103470442">changes to assets test on the family home</a>, it has yet to respond to the majority of the recommendations. But given the aged care minister chaired the taskforce, it’s likely to provide a good indication of current thinking.<!-- Below is The Conversation's page counter tag. We looked at 700 plant-based foods to see how healthy they really are. Here’s what we found

<p><em><a href="https://theconversation.com/profiles/laura-marchese-1271636">Laura Marchese</a>, <a href="https://theconversation.com/institutions/deakin-university-757">Deakin University</a> and <a href="https://theconversation.com/profiles/katherine-livingstone-324808">Katherine Livingstone</a>, <a href="https://theconversation.com/institutions/deakin-university-757">Deakin University</a></em></p> <p>If you’re thinking about buying plant-based foods, a trip to the supermarket can leave you bewildered.</p> <p>There are plant-based burgers, sausages and mince. The fridges are loaded with non-dairy milk, cheese and yoghurt. Then there are the tins of beans and packets of tofu.</p> <p>But how much is actually healthy?</p> <p>Our nutritional audit of more than 700 plant-based foods for sale in Australian supermarkets has just been <a href="https://www.sciencedirect.com/science/article/pii/S0889157524000516">published</a>. We found some products are so high in salt or saturated fat, we’d struggle to call them “healthy”.</p> <h2>We took (several) trips to the supermarket</h2> <p>In 2022, we visited two of each of four major supermarket retailers across Melbourne to collect information on the available range of plant-based alternatives to meat and dairy products.</p> <p>We took pictures of the products and their nutrition labels.</p> <p>We then analysed the nutrition information on the packaging of more than 700 of these products. This included 236 meat substitutes, 169 legumes and pulses, 50 baked beans, 157 dairy milk substitutes, 52 cheese substitutes and 40 non-dairy yoghurts.</p> <h2>Plant-based meats were surprisingly salty</h2> <p>We found a wide range of plant-based meats for sale. So, it’s not surprising we found large variations in their nutrition content.</p> <p>Sodium, found in added salt and which contributes to <a href="https://www.heartfoundation.org.au/bundles/healthy-living-and-eating/salt-and-heart-health">high blood pressure</a>, was our greatest concern.</p> <p>The sodium content varied from 1 milligram per 100 grams in products such as tofu, to 2,000mg per 100g in items such as plant-based mince products.</p> <p>This means we could eat our entire <a href="https://www.betterhealth.vic.gov.au/health/healthyliving/salt">daily recommended sodium intake</a> in just one bowl of plant-based mince.</p> <p>An <a href="https://www.tandfonline.com/doi/full/10.1080/09637486.2022.2137786">audit</a> of 66 plant-based meat products in Australian supermarkets conducted in 2014 found sodium ranged from 316mg in legume-based products to 640mg in tofu products, per 100g. In a <a href="https://www.mdpi.com/2072-6643/11/11/2603">2019 audit</a> of 137 products, the range was up to 1,200mg per 100g.</p> <p>In other words, the results of our audit seems to show a consistent trend of plant-based meats <a href="https://www.tandfonline.com/doi/full/10.1080/09637486.2022.2137786">getting saltier</a>.</p> <h2>What about plant-based milks?</h2> <p>Some 70% of the plant-based milks we audited were fortified with calcium, a nutrient important for <a href="https://www.betterhealth.vic.gov.au/health/healthyliving/calcium">bone health</a>.</p> <p>This is good news as a <a href="https://www.mdpi.com/2072-6643/12/5/1254">2019-2020 audit</a> of 115 plant-based milks from Melbourne and Sydney found only 43% of plant-based milks were fortified with calcium.</p> <p>Of the fortified milks in our audit, almost three-quarters (73%) contained the <a href="https://www.eatforhealth.gov.au/food-essentials/five-food-groups/milk-yoghurt-cheese-andor-their-alternatives-mostly-reduced-fat">recommended amount of calcium</a> – at least 100mg per 100mL.</p> <p>We also looked at the saturated fat content of plant-based milks.</p> <p>Coconut-based milks had on average up to six times higher saturated fat content than almond, oat or soy milks.</p> <p><a href="https://www.mdpi.com/2072-6643/12/5/1254">Previous audits</a> also found coconut-based milks were much higher in saturated fat than all other categories of milks.</p> <h2>A first look at cheese and yoghurt alternatives</h2> <p>Our audit is the first study to identify the range of cheese and yoghurt alternatives available in Australian supermarkets.</p> <p>Calcium was only labelled on a third of plant-based yoghurts, and only 20% of supermarket options met the recommended 100mg of calcium per 100g.</p> <p>For plant-based cheeses, most (92%) were not fortified with calcium. Their sodium content varied from 390mg to 1,400mg per 100g, and saturated fat ranged from 0g to 28g per 100g.</p> <h2>So, what should we consider when shopping?</h2> <p>As a general principle, try to choose whole plant foods, such as unprocessed legumes, beans or tofu. These foods are packed with vitamins and minerals. They’re also high in dietary fibre, which is good for your gut health and keeps you fuller for longer.</p> <p>If opting for a processed plant-based food, here are five tips for choosing a healthier option.</p> <p><strong>1. Watch the sodium</strong></p> <p>Plant-based meat alternatives can be high in sodium, so look for products that have <a href="https://www.eatforhealth.gov.au/eating-well/how-understand-food-labels/food-labels-what-look">around</a> 150-250mg sodium per 100g.</p> <p><strong>2. Pick canned beans and legumes</strong></p> <p>Canned chickpeas, lentils and beans can be healthy and low-cost <a href="https://www.heartfoundation.org.au/getmedia/71522940-decf-436a-ba44-cd890dc18036/Meat-Free-Recipe-Booklet.pdf">additions to many meals</a>. Where you can, choose canned varieties with no added salt, especially when buying baked beans.</p> <p><strong>3. Add herbs and spices to your tofu</strong></p> <p>Tofu can be a great alternative to meat. Check the label and pick the option with the highest calcium content. We found flavoured tofu was higher in salt and sugar content than minimally processed tofu. So it’s best to pick an unflavoured option and add your own flavours with spices and herbs.</p> <p><strong>4. Check the calcium</strong></p> <p>When choosing a non-dairy alternative to milk, such as those made from soy, oat, or rice, check it is fortified with calcium. A good alternative to traditional dairy will have at least 100mg of calcium per 100g.</p> <p><strong>5. Watch for saturated fat</strong></p> <p>If looking for a lower saturated fat option, almond, soy, rice and oat varieties of milk and yoghurt alternatives have much lower saturated fat content than coconut options.
